Takotsubo Cardiomyopathy: Establishing Diagnosis and Causes through Prospective Testing

Abstract: T he accompanying case report by Patel and colleagues 1 concerns recurrent takotsubo cardiomyopathy (TC) in the context of hyperthyroidism's onset. The authors deserve compliments on their interesting article and their correct phenotypic diagnosis of both TC and hyperthyroidism. Their report prompts me to comment about the importance of prospectively establishing the cause and nature of TC.The medical literature's focus on clinical conditions associated with TC seems intended to clarify causative or predisposi… Show more

“…Spasm can occur in angiographically normal coronary vessels, but more commonly occurs at the site of atherosclerotic plaques of variable severity. Some authors documented spontaneous or induced coronary artery spasm in stress cardiomyopathy, suggesting the possible causative role of the sustained epicardial coronary artery into pathogenesis (84) by including it in the spectrum of vasospastic angina. Different from stress cardiomyopathy, vasospastic angina is characterized by episodes of angina at rest during the night or early in the morning of much shorter duration, and a history of repetitive episodes over a period of weeks or months associated with diffuse ST-segment elevation.…”

“…Positive Ach test results are accompanied by transient, critical diffuse coronary narrowing of the TC-related territories. 5,6 The administration of intracoronary nitroglycerin usually abolishes the patient's chest pain, electrocardiographic changes, and apical ballooning within seconds.…”

“…The relationship between triggering factors and TTS naturally caused accusation of catecolamine excess in the pathogenesis of TTS [13]. Although catecolamine excess hypothesis in TTS was supported by several times higher concentrations of plasma catecholamines including epinephrine, norepinephrine and dopamin in the acute phase of TTS when compared with levels in STEMI and also increased local myocardial release of catecholamine, benefit of b-blockers were derived from trials including patients with heart failure rather than patients with TTS [14].…”
