Teaching the nursing process

Abstract: Gerard Carton, Dave Langton and David Robinson discuss a pilot study of an innovative approach to the design and implementation of a research-led, clinically-based and clinically-relevant teaching package for individualised care. The difficulties in transferring classroom-taught theory to psychiatric practice are considered. Nursing staff in the context of Rampton Hospital have, for example, experienced problems in applying the principles of the Open University OU P553 Course to the clinical setting.

A care plan audit was carried out as part of an action research project involving the introduction of primary nursing. The audit tool was based on the Roper, Logan and Tierney Activities of Living model and the nursing process. The audit showed that few changes in documentation had taken place as a result of the introduction of primary nursing. The volume of communications had increased but much of this was not documented on care plans. Other positive changes as a result of introducing primary nursing were found, and both patients and nurses were aware of these.

The nursing process was introduced to develop the contribution of nursing and adopt a more systematic approach to care delivery. Few in-depth studies have yet described its true impact and level of implementation. The study reported in this article examines the status of individualised care and makes comparisons with nursing care in other locations. It also describes the nature and context of forensic/psychiatric care and its relationship with findings of two decades ago [ NT Research 1996; 1: 1, 13-21 ]
