“…The four neighborhood coordinators provide an ongoing presence in each neighborhood and maintain relationships with residents, leaders, agency representatives, and health care professionals. One resident-driven solution to promote access to health care in each neighborhood involved the creation of a community health worker (CHW) program which was instituted in 2004 (Zandee, Bossenbroek, Friesen, Blech & Engbers, 2010; Zandee, Bossenbroek, Slager & Gordon, 2013).
