Temporary Vena Cava Filters and Ultrahigh Streptokinase Thrombolysis Therapy: A Clinical Study

Abstract: Fatal pulmonary embolisms as a complication of ultrahigh-dose treatment of pelvic or caval thrombosis can not safely be prevented by the temporary vena cava filters currently available. Filter design needs to be improved.

“…Since thrombolysis results in signifi-cantly greater reductions in clot burden than anticoagulation alone [192], there has been considerable interest in wider application of thrombolysis among patients with massive deep venous thrombosis. However, thrombolysis of ilio-caval thrombi has been associated with a significant incidence of fatal PE (~6%) by some investigators [193][194][195] although not others [196,197]. These experiences have prompted employment of temporary filters for prophylaxis during thrombolysis of ilio-caval clots [198].…”

“…The use of IVC filters during thrombolysis of iliocaval venous thrombus is a controversial subject. Thrombolysis of iliocaval thrombi has been associated with a significant incidence of fatal PE (up to 6%) by some investigators 38,39 but not reported by others. 40 Although the data are predominately observational, the evidence suggests that there is a real risk of PE associated with thrombolysis of central venous thrombi, particularly caval thrombi.…”
