Objectives: Placental alkaline phosphatase (PALP) is synthesized in placenta and increases with gestational age. Alkaline phosphatase supports pregnancy and could play an essential role in nutrient supply and growth of the fetus. Preeclampsia is a systemic disorder which affects 5 to 7 percent of women worldwide and is a major cause for maternal and neonatal morbidity and mortality. As it has a major role in fetal growth, nutrition and defense mechanism study of alkaline phosphatase enzymatic activity becomes essential. Methods: 50 normal and 50 preeclamptic placentae were collected immediately after delivery from Department of Obstetrics and Gynecology. Placentae were obtained from known preeclamptic consenting cases who had no history of hypertension before pregnancy or during first 20 weeks of gestation, who had consistently recorded systolic and diastolic blood pressure of 140 / 90 mm of Hg or above and proteinuria ≥ 300mg / 24 hrs. Alkaline phosphatase activity was demonstrated by using modified Gomori's method. Results: Intensity of PALP localization was stronger in preeclamptic placentae as compared to normotensive placentae. Conclusion: Placental ischemia is evident in preeclampsia. Uteroplacental insufficiency leads to increased syncytial damage in preeclamptic placentae which may lead to abnormally high PALP activity and consequent increase in serum alkaline phosphatase.

(1) Total alkaline phosphatase (TP) and heatstable alkaline phosphatase (HSP) activity was examined in the blood plasma during the terminal six weeks of pregnancy. Progressively rising levels of TP and HSP were observed in each of 22 healthy women and of 19 women with abnormalities during pregnancy. (2) Diabetes mellitus in pregnancy is associated with lower plasma levels of HSP and TP than are found at the same period of gestation in normal pregnancy. Three patients with pre-eclamptic toxaemia had higher than normal levels of plasma HSP. (3) Owing to its low and variable rate of metabolism plasma HSP has no value as a test of placental function.

The human placenta was found to contain a cytosol receptor for glucocorticoids. The concentration of this receptor in term placenta was 27-fold higher than that found in cytosol from first trimester placenta. The levels of cytosol glucocorticoid receptor in three trophoblastic cell lines (JAr, BeWo, and JEG) were also determined and all were found to be low. The ability of prednisolone, a potent glucocorticoid, to stimulate heat-stable alkaline phosphatase activity found in these cells was tested. Although control experiments demonstrated that the conditions were adequate to stimulate HeLa cell alkaline phosphatase, none of the trophoblastic lines responded to prednisolone administration. This result may be explained by the observation that the JAr cells lacked any detectable glucocorticoid receptor and the receptor levels in cytosol prepared from JEG and BeWo cells were 12% and 2%, respectively, of those measured in HeLa cytosol. Our studies also suggest that the increase in serum levels of heat-stable alkaline phosphatase observed during pregnancy may reflect increasing placental sensitivity to glucocorticoids as a result of increased receptor levels.
