Background The Discharge Planning Curriculum (DPC) is a 4-hour course for penultimate-year medical students at a tertiary teaching hospital in Taiwan. The course begins with a 30-minute introduction, followed by the students shadowing discharge planning case managers as they visit patients on the wards. After patient visits, the students engaged in a 1-hour case discussion. Our research assessed the effectiveness of the DPC for medical students. Methods This mixed methods study recruited medical students participating in family medicine rotations between October 2017 and May 2018. To determine the impact of the DPC, we quantitatively analyzed questionnaire responses to measure changes in self-efficacy, attitude toward discharge planning, and course satisfaction before and after completing the DPC. Additionally, we conducted qualitative focus group interviews to gain insight into the students’ learning experiences and applied thematic analysis to the interview data. Results Our study found two quantitative results: 1) The DPC significantly improved self-efficacy and attitude toward discharge planning (p < 0.001). 2) The medical students acquired knowledge about home care, assistive device application, long-term care facility referral, home rehabilitation, and home care services, and more than 95% of the students reported being satisfied with the course. In addition, the focus group interviews revealed that medical students learned several aspects of discharge planning through the curriculum, including an understanding of various aspects of discharge planning through the curriculum, the importance of early discharge planning, the roles of doctors and case managers, and the challenges faced in the process. Conclusion The DPC helped medical students understand different team members’ roles in discharge planning and appreciate the challenges that case managers face in this process. The DPC improved medical students’ attitudes toward discharge planning. The curriculum can be a valuable tool in training future healthcare providers in effective transitional care.

Implementation of discharge planning in children with ARI (Acute Respiratory Infection) in Indonesia is one of the efforts to prevent recurrent ARI in children. This study aimed to identify the implementation of discharge planning in children with ARI in Indonesia. A literature review searched four databases (Google Scholar, Science Direct, PubMed, and Springer) for all studies design between 2016-2021. The Prisma checklist guided in this review, title, abstract, full-text, and methodology were assessed for the eligibility of studies. The researcher found ten studies that met inclusion criteria in the review. Included studies consist of three studies discussing the factors that cause ARI, five studies discussing the implementation of discharge planning, and two studies of both. The average number of participants is 95 and most of the study design in this study used cross-sectional. 1) the importance of knowledge about the risk factor of ARI for discharge planning; 2) the risk factors of ARI in children, such as nutrition status, birth weight, exclusive breastfeeding, parental smoking status, dwelling density, ventilation, and humidity; 3) the implementation of discharge planning in the pediatric room was unoptimally implemented; 4) factors that causing unoptimally implementation of discharge planning are knowledge, competency, and coordination of nurse. Discharge planning is an important and crucial process in patient’s service transition. Well planned discharge planning will increase patient’s flow. At Dr Kariadi Central General Hospital, according to the evaluation of the patient's discharge in September 2021, it was found that only 13% of patients had planned discharges. A qualitative study was conducted from January to March 2022. This study aims to know the implementation of the day before discharge program at Dr. Kariadi Central General Hospital. The subject was patient from 1st and 2nd class inpatient installation and 3rd class inpatient unit - stroke unit of Dr. Kariadi Central General Hospital Semarang. Data were obtained from in-depth interviews with all elements involved in planning the day before discharge, observation, and document review. Based on the result of our study, implementation of one day before discharge plan at Dr. Kariadi Central General Hospital Semarang has been unconsistantly increased (to 77,73% on February 2022, 76,45% on march 2022, and 77,69% on April 2022), even though it has not achieved the expected goal yet (more or equal to 80%), several factors influenced the implementation of patient discharge process, such as regulations, doctors and assistants (regarding doctor’s visite schedule), patient and family, and communication between doctor, nurse, other medical staff, and patient. Our study concluded that one day before discharge plan at Dr. Kariadi Central General Hospital Semarang can be improved.
