The Anterior and Medial Reduction of Intertrochanteric Fractures: A Simple Method to Obtain a Stable Reduction

Abstract: This article describes a simple method using anterior and medial bone contact of the distal shaft piece to obtain a stable reduction for displaced intertrochanteric fractures of the hip. The technique is based on the observation that the anterior pathoanatomy of the intertrochanteric hip fracture involves a noncomminuted fracture plane that can be placed into contact. This reestablishes a normal neck shaft angle and prevents excessive collapse of the fracture site. The maneuver is described in the context of t… Show more

“…However, the study did not discuss whether displacement was in the anterior or posterior direction. An anatomical characteristic of the femoral trochanteric region is that the posterior cortex is thin and fragile, and intensity may not be suYcient when impaction is placed excessively on this area [8]. Because the femoral neck is anteverted, the lag screw is inserted in a posteroanterior direction up to the head and neck fragment.…”

“…In the trochanteric region of the femur, an anterior bone cortex is thick, and conversely, a posterior cortex is thin and often comminuted when a fracture is sustained [8]. If bone-on-bone impaction is applied primarily to the posterior cortex, the fracture site may collapse.…”

“…This may be due to better early pain relief and mobilisation, but there is also a possibility of the compression being less and the offset thus being less affected. Even without additional components improvements could possibly be achieved through better perioperative fracture reduction [31,32].…”
