Penerapan dan penelitian terkait efektivitas automated dose dispensing (ADD) di Indonesia masih sangat terbatas. Oleh karena itu, penelitian ini bertujuan untuk melihat efektivitas ADD di salah satu rumah sakit di Indonesia yang telah menerapkannya, yaitu Rumah Sakit Umum Kabupaten Tangerang. Metode penelitian yang digunakan adalah cross-sectional study dengan membandingkan jumlah dispensing error dan dispensing time dari populasi resep yang menggunakan ADD dan manual dispensing (MD). Sampel didapat dari resep unit dose dispensing (UDD) bangsal rawat inap RSU Kabupaten Tangerang periode Mei-Juli 2019. Hasil penelitian dari 1086 resep untuk masing-masing populasi menunjukkan terdapat perbedaan rata-rata geometrik dispensing time yang signifikan (p<0,001) antara ADD yaitu 53,70 (52,48-54,95) detik dengan MD yaitu 144,54 (141,25-147,91) detik. Hasil penelitian ini juga memaparkan terdapat perbedaan jumlah resep dengan dispensing error yang signifikan (p<0,002) antara ADD yaitu 44 errors dan MD yaitu 77 errors dengan nilai odds ratio sebesar 1,83. Hal ini menunjukkan bahwa di RSU Kabupaten Tangerang, ADD lebih efektif dibandingkan dengan MD dilihat dari ratarata geometrik dispensing time dan jumlah resep dengan dispensing error. Meskipun ADD dapat mempercepat dispensing time dan menurunkan jumlah dispensing error, penting dilakukan telaah sebelum menerapkan mesin ini dengan mempertimbangkan faktor biaya, sistem informasi rumah sakit, resep elektronik, dan keterampilan sumber daya manusia.

Both public and private hospitals are increasingly under pressure to improve patient care across all medical disciplines and departments. Hospitals must become patient-oriented, responsive, lean and adequate in order to properly realign and integrate health care processes. One way it is necessary to reform current systems of medical care and improve the efficiency of medical care. The subject of this study was an outpatient service in a hospital. The purpose of this study is to design improvement surgery in outpatient service in a hospital to be more efficient and effective. The research uses the approach of business process re-engineering (BPR) to improve service process time and reduce waiting time. In the BPR approach, two main steps, which are mapping as-is process. The as-is mapping model, the model is created and simulated to examine the current process time of the hospital outpatient pharmacy, and then asis model is analyzed, and then several to-be models with different scenarios is designed and compared with as-is model. The models are simulated and run using Business Process Modeling Notation (BPMN) in iGrafx software to investigate how much the healthcare services has improved compared to the current condition.

Improvement of Night Pharmacy Service by Automated Dispensing Cabinet System Implementation in Emergency Medical Center

Objective: An automated process for medication preparation and dispensing is essential to improve the quality of work. To reduce night pharmacy workload, a new automated dispensing cabinet system was implemented in a hospital emergency medical center. The purpose of this study is to verify that implementation of an automated dispensing cabinet system will influence the efficiency of night pharmacy work. Methods: To evaluate the new system implementation, a retrospective study and survey was performed in the Ewha Womans University medical center. We compared the dispensing and near-miss error rates between the automated dispensing cabinet system and a night pharmacy. The degree of satisfaction of night shift workers with the new system was surveyed. Results: This study showed significantly reduced dispensing rates of night medications (56.1% and 37.3%; p < 0.01) and near-miss night medications (0.27% and 0.17%; p<0.01). Thirty-two persons responded to the survey, and the satisfaction score for the new system was 4.0 (±0.8). The scores were high in order of efficiency, management, and convenience. Time requirement was also reduced because of the simple step of only reviewing in the pharmacy with the new system. Conclusion: Due to system implementation, workload was reduced and time was saved for not only night shift workers but also patients receiving emergency discharge medicine. It was suggested that this will have a positive effect on pharmacist medical service and patient safety.
