The blood supply of the lateral epiphyseal arteries in Perthes’ disease

Abstract: We suggest that in Perthes' disease the blood supply of the LEAs is impaired at their origin and that revascularisation occurs from this site by ingrowth of small vessels into the femoral epiphysis. This process may be the result of recurrent ischaemic episodes.

“…This relationship was described previously [8,9]. Acetabular retroversion leads to overcoverage of the femoral head by the anterior wall of the acetabulum [2,13,26], and it is the overhanging rim that theoretically could impinge in flexion and internal rotation against the retinacular vessels resulting in microtrauma to the vascular supply of the femoral head [2,26] (Fig. 4).…”

“…The etiology of the disease is unclear, although some investigators attribute the cause to an undefined vascular insult [2]. Proposed risk factors include coagulopathy, low birth weight, and maternal tobacco use [3,11].…”

“…У 68% наблюдаемых были отмечены аномалии в месте отхождения этой артерии. Авторы предполагают, что кровоснабжение латераль-ных артерий нарушается в самом начале заболевания, а прорастание мелких артерий на этом участке приводит к явлениям ишемии [15].…”

“…Yeniden kanlanma ile birlikte nekrotik kemik rezorbe olarak yerine inflamatuvar hücre-ler, fibroblast-benzeri hücreler ve osteoklastlardan L egg-Calvé-Perthes (LCP) hastalığı, femur başı epifizinin vasküler beslenmesinin bozulması sonucu epifizin bir kısmı veya tamamında avasküler nekroz gelişimidir. [1,2] Histolojik çalışmalarda, femur başı epifizinde iskemik hasara bağlı doku nekrozu gös-terilmiştir. [3] İmmatür memeli hayvanlarda femur başı kanlanmasının iyatrojenik olarak bozulduğu deneysel çalışmalarda, LCP hastalığında görülen radyolojik ve histolojik değişikliklerin benzeri oluşturulmuştur.…”
