The Conservative Surgical Treatment of Idiopathic Aseptic Necrosis of the Femoral Head

Abstract: THE JOURNAL OF BONE AND JOINT SURGERY surface involved by necrosis in antero-posterior and lateral radiographs of the head, and adding the two angles (Fig. 1). The extent was considered large when the sum was 200 degrees or more and small when 160 degrees or less. The figures so obtained were: large, 47 per cent: medium, 38 per cent; and small, 15 per cent.

“…Although numerous treatment algorithms have been proposed to provide guidance to the treating physician [18,19,45,58,64], controversy remains surrounding the most appropriate surgery and at what stage of the disease process these procedures should be performed [9,10,34,50,59,60].…”

“…The extent of necrosis was determined by the method described by Kerboul et al [13] and the exact location of the lesion was determined by MRI. This information was used to determine the rotation angle required for the modified transtrochanteric rotational osteotomy.…”

“…Preoperatively, according to the modified radiographic classification system of Ficat 17,18 , sixty-nine hips were in stage IIB and forty-four hips were in stage III. The Kerboul combined necrotic angle 20,21 ranged from 195°to 260°(mean, 218.3°) ( Table I). The preoperative hip scores according to the system of Merle d'Aubigné et al 22 ranged from 9 to 16 points (mean, 14.1 points).…”

“…We measured the extent of the osteonecrosis with a modification of the combined necrotic angle of Kerboul et al 20 as we described previously 21 . The necrotic angle was measured on magnetic resonance imaging scans instead of radiographs.…”
