Objective. The purpose of this study was to investigate the effect of kinesio taping on the walking ability in patients with foot drop after stroke. Methods. Sixty patients were randomly divided into the experimental group (with kinesio taping) and the control group (without kinesio taping). The 10-Meter Walking Test (10MWT), Timed Up and Go Test (TUGT), stride length, stance phase, swing phase, and foot rotation of the involved side were measured with the German ZEBRIS gait running platform analysis system and were used to evaluate and compare the immediate effects of kinesio taping. All the measurements were made in duplicate for each patient. Results. The demographic variables of patients in both groups were comparable before the treatment (p>0.05). After kinesio taping treatment, significant improvement was found in the 10MWT and the TUGT for patients in the experimental group (p<0.05). There were significant differences in the 10MWT and TUGT between the experimental and control groups after treatment (p<0.05). In terms of gait, we found significant improvement in stride length (p<0.001), stance phase (p<0.001), swing phase (p<0.001), and foot rotation (p<0.001) of the involved side in experimental group after treatment compared with those before treatment. Further, the functional outcomes and gait ability were significantly improved in the experimental group after treatment (p<0.05), compared to the control group. Conclusion. Kinesio taping can immediately improve the walking function of patients with foot drop after stroke.

Background and purposeUntil now, stroke and transient ischemic attack (TIA) have been clinically based terms which describe the presence and duration of characteristic neurological deficits attributable to intrinsic disorders of particular arteries supplying the brain, retina, or (sometimes) the spinal cord. Further, infarction has been pathologically defined as death of neural tissue due to reduced blood supply. Recently, it has been proposed we shift to definitions of stroke and TIA determined by neuroimaging results alone and that neuroimaging findings be equated with infarction.MethodsWe examined the scientific validity and clinical implications of these proposals using the existing published literature and our own experience in research and clinical practice.ResultsWe found that the proposals to change to imaging-dominant definitions, as published, are ambiguous and inconsistent. Therefore, they cannot provide the standardization required in research or its application in clinical practice. Further, we found that the proposals are scientifically incorrect because neuroimaging findings do not always correlate with the clinical status or the presence of infarction. In addition, we found that attempts to use the proposals are disrupting research, are otherwise clinically unhelpful and do not solve the problems they were proposed to solve.ConclusionWe advise that the proposals must not be accepted. In particular, we explain why the clinical focus of the definitions of stroke and TIA should be retained with continued sub-classification of these syndromes depending neuroimaging results (with or without other information) and that infarction should remain a pathological term. We outline ways the established clinically based definitions of stroke and TIA, and use of them, may be improved to encourage better patient outcomes in the modern era.

Sažetak. Prema Svjetskoj zdravstvenoj organizaciji moždani udar je klinički sindrom definiran kao naglo nastali žarišni ili, rjeđe, globalni neurološki deficit koji traje dulje od 24 sata ili dovodi do smrti, a može se objasniti cerebrovaskularnim poremećajem. Razlikujemo ishemijski i hemoragijski moždani udar. Moždani udar je akutno stanje koje zahtijeva ranu intervenciju i hitno liječenje. Prema epidemiološkim podacima moždani udar je vodeći uzrok onesposobljenosti u odrasloj dobi u Sjedinjenim Američkim Državama, treći je uzrok po smrtnosti u zemljama zapadne Europe, drugi uzrok smrtnosti u Hrvatskoj i prvi uzrok invalidnosti, a drugi vodeći uzrok smrti širom svijeta. Društva American Heart Association i American Stroke Association izdala su 2013. godine nove smjernice za rano zbrinjavanje pacijenata s ishemijskim moždanim udarom. Godine 2015. učinjena je nadopuna smjernica iz 2013. godine za rano zbrinjavanje ishemijskog moždanog udara, uključujući endovaskularno liječenje. Američka akademija za neurologiju priznala je ovu vrstu smjernica kao edukacijsku pomoć za liječnike neurologe. Ljestvice poput RACE-a (engl. Rapid arterial occlusion evaluation scale), FASTED a (engl. Field assessment stroke triage for emergency destination), C-STAT-a (engl. Cincinnati stroke triage assessment tool) i LAMS-a (engl. Los angeles motor scale) imaju veliku važnost u ranom, hitnom otkrivanju akutnog zbivanja u procesu nastanka moždanog udara te se koriste od tada i pridonose bržem otkrivanju i zbrinjavanju pacijenata. Od kliničke važnosti za početak liječenja i praćenja tijekom liječenja bitna nam je i NIHSS ljestvica (engl. National institutes of health stroke scale). Ljestvica NIHSS koristi se u procjeni pacijentova stanja za primjenu trombolitičke terapije u liječenju moždanog udara. Zaključno je bitno utvrditi važnost ljestvica koja se očituje u što ranijoj intervenciji te mogućnosti zbrinjavanja pacijenta u izvanbolničkim i bolničkim uvjetima unutar zlatnog sata i postizanja boljih rezultata u liječenju moždanog udara i sprječavanju njegovih posljedica.
