“…Traditional harvesting technique, however, necessitates skin grafting to the forearm donor site. A “kissing” flap design has been previously developed to increase the surface area of skin flap coverage while minimizing the donor‐site morbidity . By applying the “kissing” flap design for the RFF (KRFF), we were able to decrease the width of flap by half, allowing primary closure of donor site in all patients.…”

“…The design of the KRFF is outlined in Figure using the “kissing” principle to reduce the width of the donor defect. Two separate skin islands were outlined along the vascular pedicle with the distal island representing the dorsal tongue and the proximal island, the ventral tongue, according to the FIPELS design (Figure A,B and Supporting Information Video S2).…”

“…For reconstructive microsurgery, it is important to maximize the use of limited autologous tissue resources when performing a one‐stage reconstruction involving multiple defects in the head and neck region. Economy in free tissue transfer refers to the efficient use of limited autologous tissue to achieve the best possible outcome in function and cosmesis, and to minimize donor‐site morbidity, as well as to reduce operative time and recovery period . Based on the concept of “economy of autologous tissue transfer,” we describe these techniques here.…”

“…However, restoration of a functional circumferential hypopharyngeal track along with acceptable external neck coverage most times requires 2 separate flaps . In this series, we have applied our previously described concept of the chimeric flap and kiss flap technique for TAAP flap, as a novel, reproducible, and versatile pedicled chimeric TAAP flap design to offer a solution. This is a unique reconstructive technique that can be performed safely in selected cases and not only focuses on circumferential hypopharyngeal defect reconstruction but also addresses the problem of neck skin defects, thus expanding the indications and the applicability of the TAAP flap.…”
