Background: The use of tourniquet during TKA aside from having several advantages, is also accompanied by several risks. This study was performed to prove the relation between the duration of tourniquet application to inflammatory response, pro-thrombotic condition, and pain response of the patients undergoing TKA procedures.Methods: This study is a cross-sectional analytical experimental research. A femoral tourniquet was applied to 34 primary osteoarthritis (OA) patients undergoing the standard procedure of TKA and the duration of tourniquet application data was noted and collected. Pain assessment was performed by using Visual Analogue Scale (VAS), whereas blood samples are taken from the patients and IL-6 cytokine and D-dimer levels were identified at 12 hours and 24 hours post-operatively. Statistical analysis (Pearson correlation) was performed to see the correlation of tourniquet duration to the VAS, IL-6, and D-dimer serum level at 12 hours and 24 hours post-operatively.Results: The result of this study showed a positive correlation between the duration of tourniquet use with IL-6 at 12 hours post operation (r=0.359) and 24 hours post operation (r=0.658); with D-dimer at 12 hours post operation (r=0.491) and 24 hours post operation (r=0.483); and with VAS at 12 hours post operation (r=0.647) and 24 hours post operation (r=0.507) with p<0,05. A positive correlation was found between tourniquet time and IL-6, D-dimer and VAS.Conclusions: The duration of tourniquet cut-off time of more than 122.5 minutes elevates IL-6 and D-Dimer which increases the risk for SIRS and DVT.

Aims: This study was designed to investigate the effects of bilateral surgical ovariectomy on thermoregulation and blood constituents in adult Nubian goats. Materials and Methods: Twenty female goats were randomly assigned into 2 groups, group A served as control and group B was subjected to bilateral ovariectomy (OVX) by standard surgical method and monitored for 4 weeks. The effect of OVX on rectal temperature (Tr), respiratory rate (RR), heart rate (HR), body weight (BW) and blood components were investigated. The data were analyzed using an independent sample T-test. Results: In OVX group, both Tr and RR decreased during the experimental period. Tr decreased (p<0.05) at week 4 and (HR) (p<0.01) increased at week 2 in OVX group. The packed cell volume (PCV), red blood cell count (RBCs) and haemoglobin concentration (Hb) declined (p<0.05) in OVX group at week 1. Also, in OVX group, the total leukocyte count (TLC) increased (p<0.01) at week 1 and neutrophils ratio (p<0.05) at week 2. Both total protein and albumin levels in OVX group were decreased significantly (p<0.01) in weeks 1 and 2. Glucose level increased (p<0.001) at week 1 in OVX group. The serum levels of urea, GOT and GPT of OVX group were relatively higher compared with control group values. Conclusions: The study concluded that ovariectomy induced variable effects on thermoregulation, haematological and serum biochemical parameters in adult Nubian goats .

Резюме. Артеріальні турнікети широко застосовують у клінічній практиці при ангіо- та артропластиках, а при пораненнях кінцівки їх використання нерідко відбувається за життєвими показаннями. Відновлення кровопостачання кінцівки після зняття турнікета може викликати ішемічно-реперфузійні ураження як первинно ішемізованих тканин, так і у віддалених тканинах та органах. Проте на сьогодні потребують глибшого вивчення ультраструктурні зміни м’яких тканин на місцевому рівні при ішемії-реперфузії. Мета дослідження – вивчити ультраструктурні зміни скелетних м’язів задніх кінцівок щурів при гострій ішемії та наступній реперфузії, викликаній накладанням артеріального турнікета. Матеріали і методи. Проведено електронно-мікроскопічне дослідження скелетних м’язів задніх кінцівок 12 щурів за умов експериментальної гострої ішемії і наступної реперфузії. Гостру ішемію викликали шляхом накладання на задні кінцівки тварин гумових джгутів SWAT шириною 5–6 мм, на рівні пахвинної складки протягом 2 год під тіопентал-натрієвим знеболюванням, а реперфузію моделювали шляхом зняття турнікета. Дослідження зразків м’язової тканини проводилося на кафедрі гістології та ембріології Тернопільського національного медичного університету імені І. Я. Горбачевського МОЗ України за допомогою електронного мікроскопа ПЕМ–125К. Результати. При електронній мікроскопії скелетних м’язів задніх кінцівок щурів виявляли набряк саркоплазми міосимпласта, гіпертрофію та набряк мітохондрій, нерівномірне їх розміщення зі скупченням під сарколемою, ушкодження їх зовнішньої оболонки, часткова дефрагментація та деструкція мітохондріальних крист, аутолізис окремих мітохондрій, розволокнення міофіламентів міофібрил. Спостерігали зміни форми, розмірів та просторової орієнтації Z-дисків, саркомери не завжди чітко візуалізувалися, наявні ознаки розширення канальців саркоплазматичної сітки. Навколо ядер міофібрил спостерігали групи фагосом, ядра міосимпласта мали неправильну форму, були осміофільними і пікнотично зміненими. Ультраструктурні зміни спостерігались вже через 2 год після зняття турнікета і були найбільше вираженими через 1 добу. У пізньому реперфузійному періоді відбувався зворотний розвиток патологічних змін. Висновки. Ультраструктурні порушення ушкодженої ішемією-реперфузією м’язової тканини задніх кінцівок щурів досягали максимального вираження у щурів через 1 добу після зняття турнікета, що підтверджує розвиток у експериментальних тварин ішемічно-реперфузійного синдрому. В пізньому реперфузійному періоді відбувався зворотний розвиток патологічних змін.
