The effect of glucocorticoids on bone and muscle

Abstract: This article examines the current knowledge of the effects of both exogenous and endogenous glucocorticoids on bone and muscle. It demonstrates the similarity of effects of supraphysiologic loads of glucocorticoids regardless of whether they enter the body in the form of medication or are manufactured by the body in response to stimuli such as inflammation. The effects of endogenous glucocorticoids and the systemic inflammatory response resulting from pediatric burn injury are compared and the difficulty in so… Show more

“…Prolonged effects of elevated glucocorticoids may also increase the likelihood of mortality as they have been shown to: 1) reduce body mass through protein catabolism and/or reduced food intake (De Vos et al 1995, Schmitz et al 1997, Klein 2015, Osborne 2015; and 2) increase myopathy (Gupta and Gupta 2013). Persistently elevated glucocorticoids can also have immunosuppressing effects (Dhabhar andMcewen 1997, French et al 2010), leaving individuals vulnerable to disease.…”

“…The selection of covariates was made a priori according to expert opinion and previous literature, namely age 37,38 , sex 38 , C-reactive protein 39,40 , diabetes mellitus 41,42 , dialysis vintage 42 , time after transplant, hemoglobin 43 , and eGFR 31 . Long-term use of immunosuppressive agents including steroids and calcineurin inhibitors has a negative effect on the muscles of kidney transplant recipients 44,45 . Optimism assesses the magnitude of overfitting of logistic regression model (a value less than 0.2 is considered as good), and was calculated using C-statistics by 150 times of bootstrap sampling.…”

“…Glucocorticoids are associated with peripheral muscle wasting, which is thought to be related to complex mechanisms that include oxidative damage, stimulation of inflammatory cytokines and inhibition of muscle satellite or stem cell populations. 32 Our study also included a patient with longstanding s-IBM who presented with proximal weakness with normal muscle enzymes and muscle US findings were interesting. In this case, a mixture pattern of inflammation in right biceps brachii and atrophy in the proximal lower limb was found, especially in vasti muscles.…”
