The Effect of Oral Polio Vaccine at Birth on Infant Mortality: A Randomized Trial

Abstract: Neonates were randomized to oral polio vaccine (OPV) and followed for mortality. Among those enrolled within the first 2 days, OPV was associated with a 42% (10%–62%) reduction in infant infectious disease mortality. OPV at birth may provide nonspecific protection against infections.

“…The strongest evidence of such effects in humans relates to the apparent ability of the Bacille Calmette-Guerin (BCG) vaccine (live attenuated Mycobacterium bovis ) to reduce all-cause infant mortality in high mortality settings 5-7 (see Table 1). Similar benefits have been reported for other live attenuated vaccines, specifically measles vaccine and oral polio vaccine 8, 9 . The observed reductions in all-cause mortality appear to extend beyond the direct protection induced by these vaccines against their target pathogens, and likely reflect the induction of resistance to unrelated pathogens via enhancement of heterologous adaptive immune responses and/or antigen non-specific innate immune mechanisms.…”

“…A recent RCT evaluated the effect of a birth dose of OPV on all-cause mortality in Guinea Bissau and found no statistically significant effect of the vaccine on mortality (HR 0.83, 95% CI 0.61 to 1.13) 17. Various subanalyses found some evidence of significant NSE, particularly if the vaccine was given early, but this study does not provide conclusive evidence for an effect of OPV in reducing all-cause mortality.…”

“…Sex differential non-specific effects of vaccines are common in developing countries with negative nonspecific effects (NSE) of inactivated vaccines more common in girls than boys in some countries, but with no differences in high income countries 16,17 . Though the general pattern is both negative and positive NSE are stronger in females 18 , a randomized controlled trial in Guinea-Bissau negated the hypothesis that mortality rates in boys would be lower if they had not received OPV 19 . The reasons why OPV uptake is slightly higher for females in our study area are unknown.…”
