Purpose Of the 80% people with psychosis living in low- and middle-income countries (LMICs), up to 90% are left to the care of families. The World Health Organization has recommended the inclusion of families in community-based rehabilitation and while there is evidence of its implementation in LMICs, this has not been reviewed yet. This study aims to describe the key features and implementation strategies of family-based interventions in LMICs, and appraise their effectiveness. Methods Included are people with psychosis in LMICs who receive any form of family-based intervention, compared to their usual or absence of treatment, with patient outcome measures. We searched (August 2021) through Embase, MEDLINE, Global Health, PsycInfo, Social Policy and Practice, and Cumulative Index to Nursing and Allied Health Literature (CINAHL), as well as from grey literature and hand-searched records. Risk of bias was assessed through the Integrated Quality Criteria for Review of Multiple Study Designs (ICROMS) and Consolidated Health Economic Evaluation Reporting Standards (CHEERS), then analyzed narratively. Results 27 studies were included from the 5254 records. Psychotherapeutic features, systems approach and task-sharing were key intervention elements. Delivery strategies included preliminary research, sustained family engagement, and cultural adaptation. There were positive health impacts across four outcome domains. Conclusion All studies recommended family-based interventions, with limitations in heterogeneity and 70% of them rated high risk of bias. Other Review was registered in PROSPERO (CRD42021256856). The authors did not receive funding for this research.

Background/Objectives: Psychoeducation is a specialized form of psychological education aimed at helping people suffering from psychological problems and their families. To investigate the efficacy of psychoeducation interventions (PEIs) in improving both the burden and quality of life of caregivers and the health conditions of their adolescent or youth patients. Methods: The following databases were used: PubMed, PsycInfo, CINAHL Plus with full text, Medline and Nursing Reference Center Plus. Two search strings were developed, one for the mental health conditions of assisted patients and the other one for caregivers. Results: We selected 30 articles and applied two differentiated meta-analyses on 12 of them to evaluate the effectiveness of PEIs. We highlighted a statistically significant superior efficacy of PIEs compared to control groups in five studies in the meta-analysis of studies on caregiver outcomes, and eight studies in the meta-analysis of studies on outcomes of patients cared for. Conclusions: PEIs were shown to be effective in reducing symptoms and hospitalizations in persons cared for, improving their quality of life as well as that of their caregivers. Regarding the caregiver’s care burden, our review suggests that PEIs generally improve burden in caregivers, reducing the perception of their workload in caring for adolescent or youth persons.

BACKGROUND: As many as, 45% of orphaned children and adolescents living in orphanages experience behavioral and emotional problems. Several literature reviews on the intervention of emotional and behavioral problems in orphan children and adolescents have been widely conducted. However, there is paucity in the reviews of emotional and behavioral problem interventions in the orphan group based on the type of group approach used in published articles. AIM: This study aimed to synthesize the results on the interventions of emotional and behavioral problems in orphan children and adolescents aged 7–20 years who live in institutions or orphanages. METHODS: This research was a scoping review. This review used three databases comprising PubMed, Science Direct, and Wiley Library Online. The inclusion criteria were experimental research designs and cohort studies, published in the 2009–2019, discussed emotional and behavioral problem’s intervention in orphan, and written in English. The exclusion criteria were that the research subjects had severe mental and psychosocial disorders such as autism, mental retardation, and mental disorders. The authors used three categories of keywords; the first keyword and its synonym were “intervention, treatment.” The second keyword focused on behavioral problems, which was “behavioral difficulties,” and the third keyword focuses on population and place, “orphan, institutions orphans, orphanages.” The article selection stage is described in detail in the flow diagram of Preferred Reporting Items for Systematic Reviews and Meta-Analysis. The critical appraisal stage used the checklist from Joanna Briggs Institute. RESULTS: Ten articles were found and selected for review. The main emotional and behavioral problems experienced by orphan children and adolescents include depression, anxiety, and low self-esteem, feelings of anger, and trauma. Other problems related, functional disorders, life satisfaction, the probability of suicide, and early (prodromal) symptoms of psychosis, also arise. There were six types of interventions given to overcome emotional and behavioral problems in orphan children and adolescents, but the interventions that are most often given are cognitive therapy, educational support, complementary, and alternative medicine. The approaches in groups that are often used in this review are the support group and the teaching-learning group. The combination of two or more approaches in one intervention can increase the success or effectiveness in overcoming emotional and behavioral problems. CONCLUSIONS: Depression, anxiety, low self-esteem, feeling anger, and trauma are main problems for orphans. To address these emotional and behavior problems, it is necessary to combine several interventions and a combination of group-based approaches.
