Background: Patients’ oral health is impaired in Intensive Care units (ICUs) due to loss of consciousness, open mouth, intubation, and mechanical ventilation. Objectives: The current study aimed to compare the effects of comprehensive and combined programs on oral health in patients under mechanical ventilation. Methods: This single-blind clinical trial with three groups was performed on 90 patients with mechanical ventilation admitted to ICUs of medical centers affiliated to the Zahedan University of Medical Sciences, Iran, in 2020. Patients under mechanical ventilation were randomly divided into three groups, including those receiving a comprehensive program, those receiving a combined program, and controls through permutated blocks. In the two intervention groups, oral care was performed for 5 minutes, three times per day, during the 5 days of the study with a specific method. The control group received routine oral care. The patients’ oral health was measured once before the intervention and then every day until the fifth day of the intervention using the Beck Oral Assessment scale. The data were analyzed using the analysis of variance and chi-square in IBM SPSS Statistics software version 26. Results: The mean oral health score in the three groups was not significantly different before the intervention and on the second day of the study (P > 0.05). However, on the third to fifth days of the study, the oral health score in the two intervention groups showed a significant decrease compared to the control group, indicating an oral health improvement in these groups (P = 0.001). The mean oral health score in the control group increased during the study days. Conclusions: Comprehensive care and combination methods can improve oral health in patients under mechanical ventilation. Nurses are recommended to use regular and codified oral care methods.

Background: Oral care is vital nursing action for intubated patients that aims to reduce the mortality and morbidity among intensive care unit patients by preventing Ventilator-associated pneumonia.Aims of the study: The study aimed to assess the nursing staff's knowledge toward oral care of intubated patients before and after the implementing the educational program.Methodology: A pre-experimental (one-group pre\post-test) design is used to conduct this study which was in the period 22 \ December \ 2020 to 2 \ May \ 2021. The non-probability (purposive) sampling is used to select the study sample that composed of (27) nursing staff working in the ICU in Al-Diwaniya Teaching Hospital. To achieve the objectives of the study, the researcher constructed the study instruments consisting of the educational program and the knowledge questionnaire. The questionnaire consisted of two parts: the first part related to the demographic characteristics of the nursing staff participating in the study, and the second part which consisted of (25) multiple-choice questions distributed on (4) domains to assess the knowledge of the nursing staff towards the oral care of the intubated patients. To analyze the results, descriptive statistics (frequencies, percentage, arithmetic mean, standard deviation) and inferential statistics (ANOVA, t-test) were used.Results: The study showed that there are significant differences between nurses' knowledge scores in the post-test when compared to their scores in the pre-test (M=.082 in the pre-test, while in the post-test, M=.093). The study also showed there is a significant relationship only between nursing staffs' knowledge and their level of education.Conclusion: The study concluded that the educational program has a positive effect on the nursing staffs' knowledge regarding oral care for intubated patients in Al-Diwaniyah Teaching Hospital.Recommendations: The study recommended the necessity of using the oral care protocol in the intensive care units and urged the nursing staff to participate in educational and training courses on oral care in intensive care units.

Effect of specific training course for competency in professional oral hygiene care in the intensive care unit: a quasi-experimental study for developing a standardized learning curve

Background The development of evidence-based training standards can help improve the quality of educational programs for novice intensive care unit (ICU) nurses. This study was conducted to assess the application of a training course on competency development of nurses in relation to oral hygiene care in ICU patients and to develop a checklist for evaluating the competence performance. In addition, to achieve a certain level of oral hygiene competence, as well as to assess the relative importance of predicting factors and learning competency patterns in oral hygiene care, we used standard learning curve. Methods This quasi-experimental study with time series design was conducted on newly registered ICU nurses of a teaching hospital affiliated with Tehran University of Medical Sciences, Iran, between 2016 and 2018. In the first phase of this study, we designed a checklist to assess nurses' professional competence in oral hygiene care in three stages: before, during, and after care. Then, in the second phase, the level of competence of nurses in repeated times of oral hygiene care was determined based on checklist items and recorded in the learning curve. Results The greatest increase of oral hygiene care competency due to repetition was observed in the first and fourth times of repetition in comparison to the subsequent and previous steps. In the linear regression model, demographic variables predicted 12–19% of changes related to skill scores in repetitions. Conclusion According to the learning curve, newly registered ICU nurses can reach an acceptable competency after 6 repetitions of oral hygiene care.
