The Effectiveness of Oral Care Guideline Implementation on Oral Health Status in Critically Ill Patients

Abstract: Intubated patients need specific oral care due to the use of endotracheal tubes. An oral nursing care guideline needs to be implemented to guide nurses in oral care in intubated patients. To test the effectiveness of oral nursing care guideline implementation. The Rogers' Diffusion of Innovations Theory was used to introduce an oral nursing care guideline to 28 nurses working in an intensive care unit in a hospital within 2 months, using mass and private communication within a hospital management system. “…Oral care is vital for patients admitted to ICUs and can have a significant impact on clinical outcomes and these patients' health. It decreases the microbial colonization and aspiration of infected saliva (16). However, research has shown that there is no specific protocol for how to do it, how often and what equipment is needed, and nurses use different methods and tools depending on, depending on their experiences or commonly used methods inwards (12).…”

“…Oral care measures should be managed to prevent oropharyngeal microbes from colonization (7) . Gupta et al (2016) reported that providing effective oral care to ICU patients could contribute to reducing the occurrence of VAP by 60% among these patients (8) .…”

“…Many factors and barriers can affect the quality of oral care for intubated patients including nurses' knowledge, where Khasanah et al (2019) stated that knowledge is essential to maintain adherence to routine oral care for intubated patients (7) . Lin et al (2011) reported that although the nursing staff has knowledge about the risks of VAP but is ignorant of how to prevent it, including ignorance of the link between oral care and VAP prevention (10) .…”

“…Oral hygiene care is essential to prevent VAP in this population. Failure to perform regular and effective oral hygiene leads to increased levels and diversity of oral microflora that make up dental plaque [ 36 ]. Plaque on tooth surfaces, coated tongues, and periodontal disease exacerbate the patient's clinical condition as they provide a favorable environment for the growth of Gram-negative bacteria [ 37 , 38 ].…”
