The effects of isoflurane anesthesia and mechanical ventilation on renal function during endotoxemia

Abstract: The results of the current study suggest that isoflurane anesthesia (minimum alveolar concentration 1.0) with mechanical ventilation aggravates renal dysfunction during 48 h of endotoxemia and does not significantly reduce the inflammatory response or signs of tubular damage.

“…Septic AKI is generally believed to be due to regional hypoperfusion causing renal ischemia [2]. However, recent experimental reports indicate that AKI may develop even though renal blood flow and blood pressure remain within physiological limits [3][4][5]. Thus, it is possible that the pathogenesis of septic AKI is complicated by factors other than ischemia.…”

“…The cytokine response is a key link of the early inflammatory response. With the accumulation of large amounts of inflammatory cytokines, lung injury during the peri-operative period is mainly an acute process (7). Expression of pro-inflammatory cytokines and imbalance of pro-inflammatory/anti-inflammatory cytokines has an essential role in lung injury during the peri-operative period (8).…”

“…Anaesthesia with isofluorane is known to have specific effects on hemodynamics (blunting of the tachycardic response, hypotension, decreased cardiac output) and on the kidney (oliguria, leukocyte activation, decreased creatinine clearance, decreased sodium excretion, decreased renal blood flow) [29,30]. It is unknown how such effects might influence the impact of live E. coli infusion on renal ATP levels.…”
