Tirzepatide is a novel once-a-week dual glucose-dependent insulinotropic polypeptide (GIP) and glucagon-like peptide-1 (GLP-1) receptor agonist, recently approved for type 2 diabetes mellitus (T2DM) and obesity. A systematic review of the literature published in multiple meta-analyses on Tirzepatide with emphasis on its effect on glycaemic and non-glycaemic parameters was conducted. We systematically searched the electronic databases PubMed and Google Scholar up to August 2023 for meta-analyses that compared Tirzepatide with placebo or active antihyperglycaemic drugs in subjects with T2DM. Various parameters for efficacy and safety, with their point estimates and confidence intervals, such as glycated haemoglobin (HbA1c), fasting serum glucose (FSG), body weight, lipid, and cardiovascular outcomes were assessed. Six meta-analyses fulfilled the pre-specified criteria and were included in the study. In all the studies, Tirzepatide treatment at different doses resulted in a significant reduction in HbA1c and FSG levels along with a significant reduction in weight compared with active control and placebo groups. Tirzepatide significantly reduced levels of triglycerides and increased high-density lipoprotein (HDL) cholesterol, whether used as monotherapy or add-on therapy. The studies suggested the cardiovascular safety of Tirzepatide as there was no increase in major adverse cardiovascular events (MACE). The drug shows lesser hypoglycemia but predominant gastrointestinal adverse effects such as nausea, vomiting, and diarrhoea. In conclusion, Tirzepatide shows superior glycaemic control and weight loss in patients with T2DM with beneficial effects on lipids, without an increased risk of hypoglycemia and cardiovascular events.

Rates of obesity have risen over the past few decades. Subsequently, the popularity of the pharmaceutical weight-loss drug market has grown over the past few years to meet growing demand. Among the most commonly prescribed drugs for weight management, many are glucagon-like peptide-1 receptor agonists (GLP-1 agonists) which are also utilized for the management of type 2 diabetes. There is a substantial and growing body of research comparing the efficacy of different clinical trials and examining long-term safety. This literature review examines the rise of off-label prescribing practices in the management of weight, with a focus on GLP-1 agonists. Physicians and patients should be aware of the unique aspects of existing treatment options, the impacts of off-label prescribing, and the effects of these medications. This review emphasizes the importance of informed decision-making, as well as the need for further research to guide future clinical practice.

Hyperprolactinemia status is commonly known as abnormal levels of prolactin hormone inthe blood due to endocrine disorder. So far, there is no evidence explaining whetherhyperprolactinemia affected Pancreas gland and vitamin D level in male. Therefore, this study aimedto eliminate hyperprolactinemia affecting the Pancreas gland by treating it with vitamin Dsupplements. Eighteen male rats were equally divide into three groups: The first group (6 rats):received normal saline for 42 days. The second group (6 rats): rats were given 5 mg/kgmetoclopramide by intraperitoneal injection for hyperprolactinemia induction for 14 days. The thirdgroup (6 rats): hyperprolactinemic rats received 2.5 mg/kg vitamin D3 for 28 days.After the end of experimental) 42days(,hormonal parameters (prolactin, insulin, and vitamin D3)were measure, and the Pancreas gland was remove for routine paraffin-embedded section stainingwith hematoxylin and eosin staining. The result of the study revealed a significant decrease (P≤0. 01)in prolactin hormone concentration and a significant increase (P≤0. 01) in D3 concentration andinsulin hormone Group 3compared with Group2. Histological examination of parts of the pancreasgland treated with vitamin D showed a remarkable recovery in the size and number of cells, and thecells became abundant in the cytoplasm. It was also noted that the nuclei returned to a size close tonormal and took on a regular, spherical shape and central location compared to thehyperprolactinemia group. The study concluded that vitamin D had a protective effect on Pancreasgland by stabilizing insulin hormone level and restoration histological architectures throughoutpancreas gland.
