The forearm fillet flap: ‘spare parts’ reconstruction for forequarter amputations

Abstract: Radical forequarter amputation is often performed for recurrent proximal extremity tumors. A free forearm fillet flap is used to provide excellent coverage of the resultant defect without donor site morbidity. Use of a free flap from the distal portion of the extremity with proximal tumor burden is safe and effective.

“…However, both patients in this study underwent resection because of recurrence, with the same poor prognosis as in other reports. 5,6,19 No local recurrence was observed in either of the 2 cases in this study, which is also similar to previous literature. Therefore, the use of fillet flaps in these patients is oncologically sound.…”

“…The possibility of recurrence due to flaps taken from distal to the tumor cannot be denied. “…Fillet flaps can be pedicled or free flaps. Fillet flaps are described more often in the literature on lower limb amputation than upper limb amputation [ 5 , 6 ].…”

“…This can be reconstructed with skin graft, regional flaps, or free flaps from an uninvolved part of the body. The filet of the forearm free flap provides robust coverage of the amputation site with no donor site morbidity 1 . While not the standard of care, the filet flap technique is effective when primary closure is not possible if the tumor is assuredly not involving the forearm.…”
