The Holistic Coronary Physiology Display: Calculation of the Flow Separation Index in Vessel-Specific Individual Flow Range during Fractional Flow Reserve Measurement Using 3D Coronary Reconstruction

Abstract: In order to make optimal decisions on the treatment of atherosclerotic coronary heart disease (CHD), appropriate evaluation is necessary, including both the anatomical and physiological assessment of the coronary arteries. According to current guidelines, a fractional flow reserve (FFR)–based clinical decision is recommended, but coronary flow reserve (CFR) measurements and microvascular evaluation should also be considered in special cases for a detailed exploration of the coronary disease state. We aimed to … Show more

“…Hemodynamic calculations can be used that combines intracoronary pressure data and 3D anatomical parameters. The results of 3D angiographic reconstruction from the selected two angiograms and the hyperemic and resting pressure data are the basis of the simple hemodynamic equations calculating the ratio of the hyperemic and resting flow (CFR p−3D ) ( 13 , 14 ).…”

“…Based on the hyperemic and the resting pressure data, simple hemodynamic equations calculate the resting and hyperemic flow, the CFR and the MRR (right lower panel). The pressure-flow relation is displayed by our freely available software https://coronart.hu/ in patient-specific flow range (upper middle panel) ( 13 , 14 ). The CFR p−3D /FFR index can be expected to be very close to the calculated MRR value (in this example: 2.30 vs. 2.26, respectively).…”

“…The role of microvascular injury has arisen both in the acute phase and in the post-COVID cardiac syndrome provoked by possible immunemediated mechanisms [17,18]. Such a diagnostic approach that can provide information on the microvasculature without having to apply further invasive device testing during a routine FFR measurement may serve as good guidance for follow-up on COVID-19-related microvascular abnormalities [19].…”

“…In this case, the FFR value is limited due to possible microcirculation disturbances and measurement errors caused by abnormal blood flow within the CEA. To extend coronary physiological evaluation, coronary flow reserve (CFR) measurement, and flow separation index (FSi) calculation can give detailed information about the microvascular coronary status, and can provide an additive value in the functional assessment of ectasia and other slow coronary flow situations [ 5 , 6 ]. Patients with CEA were most often excluded from clinical trials and so far no guidelines cover ectasy of the coronary arteries, so making decisions about the optimal treatment of these patients is extremely difficult.…”
