The importance of minor salivary gland biopsy in sjögren syndrome diagnosis and the clinicopathological correlation

Abstract: Objective: minor salivary gland biopsy is one of the objective tests used in the diagnosis of Sjögren syndrome. The aim of our study was to compare the clinical and laboratory data of primary and secondary Sjögren syndrome cases with a lymphocyte score 3 and 4 in the minor salivary gland biopsy. Material and Method:Data from a total of 2346 consecutive minor salivary gland biopsies were retrospectively evaluated in this study. Clinical and autoantibody characteristics of 367 cases with lymphocyte score 3 or 4 … Show more

“…Moreover, inter-and intra-rater agreements may be low for pathologists, which is due to the use of different criteria for diagnosis and variable interpretations of scoring of foci. Thus, biopsy of the minor salivary glands of the lips can only serve as a gold standard of diagnosis for experienced and calibrated pathologists, who use the criteria with utmost precision [14,[29][30][31][32]. However, review of diagnostic criteria is among the most important challenges for pathologists.…”

“…In this process, the results of laboratory tests matching the reduction in salivary flow rate can indicate Sjogren's syndrome even without histopathological confirmation of focal lymphocytic infiltration of salivary glands. It should be noted that antibody titers of patients with Sjogren's syndrome are correlated with the degree of lymphocytic infiltration [28,[31][32][33][34][35][36].…”

“…Histological salivary tissues were scored as follows: Grade 0, tissues without mononuclear infiltration; Grade 1, tissues with dispersed lymphocytic infiltration; Grade 2, with moderate infiltration; Grade 3 with one or more focus groups of 50 or more lymphocytes [ 26 ].…”

“…Two years later, during a regular visit, and according to the ACR criteria and the presence of an RA diagnosis, she was diagnosed with secondary Sjögren's syndrome [9,10] . Oral (saliva substitutes) and ocular (serum tear drops, topical corticosteroids) therapies were initiated, with persistence of 200 mg/day of hydroxychloroquine, and 10 mg/week of Methotrexate [11] , 100 mg/day L-thyroxin, and oral and ocular topical treatment.…”
