“…Bu vakaların birçoğunda kateter yerleştirilmesinin hemen ardından çekilen direk grafide kılavuz tel fark edilememiş, tekrarlanan kontrol grafileri sonrası veya hastanın semptomatik olması ile komplikasyonun farkına varılabilmiştir (10,11).İşin ilginç tarafı vakaların 1/3'ünde yapılan hata işlem sırasında fark edilmektedir.
