The influence of measles vaccination on the incidence of otosclerosis in Germany

Abstract: The pathologic process of otosclerosis is characterized by an inXammatory lytic phase followed by an abnormal bone remodeling at very speciWc sites of predilection. There is a clear genetic predisposition with about half of all cases occurring in families with more than one aVected member.

“…However, there have been conflicting results as in the study by Grayeli et al [49], which could not detect measles virus RNA in the otosclerotic bone. According to the study of Arnold et al [50] and Niedermeyer et al [51,52], anti-measles vaccination seems to be correlated with decreasing numbers of stapes surgery as well as increasing age of patients at the time of surgery. In conclusion, a great body of evidence suggests that otosclerosis is an inflammatory disease associated with persistent measles virus infection.…”

“…It is well known that vaccination against measles provides a milder seropositivity (serum anti-measles IgG level) than natural infection [ 82 , 86 , 87 ]. In populations of different countries, as high as >20 % of the vaccinated population exhibited negligible anti-measles IgG serum levels, which may indicate the necessity of a revaccination [ 82 ]. The low response rate to vaccination makes possible subclinical measles infection that can be the cause of a persistent measles virus presence in the affected individuals.…”

“…Recently, we published our results of all patients that were hospitalized in Germany for otosclerosis from 1993 to 2004 [47] . Summarizing, we analyzed data obtained from the Federal Office of Statistics (Wiesbaden, Germany) based on the International Classification of Diseases (ICD) code (ICD-9: 387; ICD-10: H80) which defines otosclerosis unequivocally.…”
