The mesh repair: Tension free alternative on dealing with nerve gaps-experimental results

Abstract: We evaluated the feasibility of external epineurial splinting as a way of alleviating tension caused by sutures in the reconstruction of peripheral nerve injuries, utilizing Wistar rat median nerve injury on 40 animals, in four experimental groups with 10 animals on each surgical setting. The nerve regeneration outcomes of four surgical procedures were compared: 1) primary end-to-end sutures (EES); 2) alleviated tension sutures (ATS) with a removal of 7 mm nerve segment, namely external epineurial splinting, u… Show more

“…Our article describing ''the alleviated tension concept'' preceded some other papers that appeared in the subsequent decade 4,5 and now in this century. 1,2 We congratulate with Kechele et al for his scientific demonstration that epiperineurial staying sutures can close some critical gap distances between the stumps of transected nerves allowing a direct end-to-end suture without tension and a good functional result. However, the propylene mesh is a potential factor of fibrosis that probably could be avoided using as a good alternate the surrounding tissue.…”

“…by Kechele et al 1 and Bertelli et al 2 in which they presented the alleviated tension sutures technique using an external epineurial splint to overcome a 7-mm nerve gap in the median nerve of rats 1 and a mean of 28-mm nerve gap in the median nerve of six patients. 2 In 1987, we described a similar method for repairing nerves in five patients; the epiperineurial stitches used were 6-0 to 8-0 according to the size of the nerve under repair and were secured, instead that at a mesh, directly to the surrounding fascia tissue.…”
