The Nordic SentiMag trial: a comparison of super paramagnetic iron oxide (SPIO) nanoparticles versus Tc99 and patent blue in the detection of sentinel node (SN) in patients with breast cancer and a meta-analysis of earlier studies

Abstract: The aim of the study is to compare the efficacy of SPIO as a tracer in sentinel node biopsy (SNB) in breast cancer with Tc and patent blue in a multicentre prospective study and perform a meta-analysis of all published studies. It also aims to follow skin discoloration after SPIO injection and describe when and how it resolves. Totally 206 patients with early breast cancer were recruited. Tc and patent blue were administered in standard fashion. Patients were injected with SPIO (Sienna+) preoperatively. SNB wa… Show more

“…The noninferiority of SLNB using a magnetic substance and a magnetic probe has been verified in the French Sentimag, Nordic Sentimag trial, Sentimag Multicenter Trial, and Central‐European Sentimag trial, as well as other studies . In those studies, patients received injections of both SPIO and 99m Tc, with or without blue dye.…”

“…A short incision was made in the axilla over the area with the greatest uptake, and the sentinel node was sought for primarily using the SentiMag probe. The brownish coloring of the sentinel lymph node when SPIO is used can be an aid intraoperatively to detect sentinel lymph node for histopathology (68 (76) reported interstitially administered SPIO can be used both for pre-operative imaging and intra-operative sentinel lymph node biopsy, with equal performance to imaging and localization with radioisotopes. They described that it is a feasible technique for pre-operative localization of sentinel lymph nodes and, in combination with intra-operative use of a handheld magnetometer, provides an entirely radioisotope-free technique for sentinel lymph nodes biopsy.…”

“…The sentinel node detection rates per patient range from 94.4 to 98% [8,9,10,11,12,13,14]. A recent meta-analysis [14] including 7 non-randomised studies [8,9,10,11,12,13,14]showed that Sienna+ is non-inferior to standard mapping techniques for sentinel node detection.…”

“…A recent meta-analysis [14] including 7 non-randomised studies [8,9,10,11,12,13,14]showed that Sienna+ is non-inferior to standard mapping techniques for sentinel node detection. There was no difference in detection rate per patient (fixed odds ratio (OR) 1.10; 95% confidence interval (CI) 0.67-1.79; p = 0.71) between Sienna+ and the standard tracer technique.…”
