Background: Epilepsy is a disorder that affects 1% of the global population. It is the second most common serious neurologic disorder after stroke, affecting humans. Since antiepileptic drugs have a narrow therapeutic index and their adverse effects can affect any organ, their widespread use has significant safety implications. Objectives: The study assessed adverse drug reactions (ADRs) using antiepileptic drugs in the Department of Neurology at a Tertiary Care Hospital, Srinagar, Jammu & Kashmir, India. Methods: This prospective observational study was conducted in the Department of Neurology of a Tertiary Care Hospital, Srinagar, Jammu & Kashmir, India, for eight months. It was a spontaneous reporting of ADRs by practicing physicians in the outpatient and inpatient settings that were included in the study. Results: Of the 3,300 patients who were on the anti-epileptic drug (AED), 92 (3.07%) had AED-related ADRs. A total of 18 cases were reported in the inpatient department and 74 cases in the outpatient setting. The most common ADRs were loss of appetite (34.78%), skin rashes (17.39%), and gum hypertrophy (9.78%). Of 80 ADRs, 42.5% were related to valproate, followed by phenytoin, carbamazepine, and levetiracetam. The suspected drug was changed in 22 patients with ADRs. Conclusions: For the early diagnosis and avoidance of ADRs, the frequent follow-up of patients on AEDs is needed to improve patient compliance with drug therapy and provide better drug therapy for avoiding associated morbidity and mortality.

Background: An examination of drug use is done in a particular setting, searching for drug use that isn't fair. At the population level, the use of psychotropic drugs for mental illness can be an effective way of identifying and tracking the level of treatment for these conditions. They also provide information on ethical drug use, informed by the currently available information on a medication's benefits and risks. Objective: A major objective of the research was to gather data on patterns of drug use in the psychiatry outpatient department of a tertiary care hospital in Srinagar, Jammu & Kashmir, India. Methodology: The drug use research was performed in a tertiary care hospital in Srinagar, Kashmir, India, in a psychiatry outpatient clinic. This research was observational, cross_sectional, and open_ended in nature. All patients who attended the Psychiatry OPD and fulfilled the inclusion criteria over the course of a six_month cycle were included in the study. Results: Over a period of six months, 600 patients from the psychiatry outpatient department of a tertiary care hospital in Srinagar, Jammu & Kashmir, India were assessed. All patients were classified into seven age groups: up to 14 years old (A), 15 to 25 years old (B), 26 to 35 years old (C), 36 to 45 years old (D), 46 to 55 years old (E), 56 to 65 years old (F), and over 65 years old (G). Clonazepam was the most commonly prescribed medication (152 cases), followed by olanzapine (132 cases), lorazepam (105 cases), and escitalopram (92 cases). Conclusion: Measuring drug use in treatment centres not only explains drug use rates and prescribing behaviour, but it also helps in the identification of causes of polypharmacy and the problems associated with it. Making a habit of setting standards and gauging the quality of clinical treatment using performance feedback should become standard clinical protocol.

Alzheimer’s disease (AD) is a significant global health challenge characterized by progressive cognitive decline and neurodegeneration. Refinements in the amyloid and tau hypotheses have deepened insights into the molecular mechanisms underlying AD, while research on neuroinflammation and genetic factors, including APOE ε4, has expanded our understanding of the disease's complexity. Diagnostic advancements include cerebrospinal fluid and blood-based biomarkers, advanced imaging techniques like PET and MRI, and innovative neuropsychological assessments, enabling earlier and more accurate detection. Therapeutically, notable progress has been made with disease-modifying treatments targeting amyloid and tau proteins, including the recent approval of aducanumab. Symptomatic treatments continue to evolve, providing better management of cognitive and behavioral symptoms. Additionally, lifestyle interventions, such as diet, exercise, and cognitive training, show promise in slowing disease progression. Emerging research in gene therapy and stem cell applications offers potential for future breakthroughs in AD treatment. Despite these advances, challenges persist, including the heterogeneity of AD, difficulties in clinical trial design and patient recruitment, and ethical considerations related to early diagnosis and treatment access. This review underscores the necessity of ongoing research and interdisciplinary collaboration to develop more effective management strategies and ultimately achieve a cure for Alzheimer’s disease. Continued efforts in these areas are crucial for transforming the landscape of AD diagnosis and treatment, offering hope for patients and their families.
