The Pins and Rubbers Traction System for Treatment of Comminuted Intraarticular Fractures and Fracture-Dislocations in the Hand

Abstract: The authors have developed a new skeletal traction system for comminuted intraarticular fractures and fracture-dislocations in the hand. The system consists of two or three Kirschner wires and rubber bands, and is easy to assemble. It is more compact and comfortable than the banjo splint, and equally effective, and it allows early motion of the affected digits. A description of the technique is followed by the clinical results of seven cases of severe articular injuries in the hand. At the time of follow-up, t… Show more

“…An alternative well-established fixator is the 'pins and rubber traction system', PRTS, described by Suzuki. It is convenient because no additional equipment is needed besides standard k-wires and orthodontic rubber bands, and it is cheap and easy to install [12]. Yet positioning and adjustment of the device is less precise, and the outcome is much more dependent on the skill of the surgeon in charge [2].…”

“…Literatürde farklı uygulama şekillerine ait örnekler vardır. [14][15][16][17][18][19] Biz, klinik uygulamalarımızda, teller ve lastik bantlarla hazırladığımız fiksatörleri kullanmaktayız. Bu yöntemde amaç, parçalı kırığı distraksiyonla indirekt olarak redükte etmek ve eklemi stabil hale getirip erken harekete başlayabilmektir.…”

“…The best chance to prevent these adverse outcomes is early reduction of the joint space and articular surface combined with early, active motion [6]. Many methods have been devised to achieve early reduction and motion, including open reduction and internal fixation [3], palmar plate arthroplasty [4], external traction pinning [10], and hemi-hamate arthroplasty (HHA) [13]. Although there is no universally accepted gold standard for treatment, HHA has become one of the most commonly performed techniques to treat these fractures.…”
