ObjectiveTo assess patients’ preferred roles in healthcare-related decision-making in a representative sample of the Portuguese population.DesignPopulation-based nationwide cross-sectional study.Setting and participantsA sample of Portuguese people 20 years or older were interviewed face-to-face using a questionnaire with the Problem-Solving Decision-Making scale.OutcomesThe primary outcome was patients’ preferred role for each vignette of the problem-solving decision-making scale. Sociodemographic factors associated with the preferred roles were the secondary outcomes.Results599 participants (20–99 years, 53.8% women) were interviewed. Three vignettes of the Problem-Solving Decision-Making scale were compared: morbidity, mortality and quality of life. Most patients preferred a passive role for both the problem-solving and decision-making components of the scale, particularly for the mortality vignette (66.1% in the analysis of the three vignettes), although comparatively more opted to share decision in the decision-making component. For the quality of life vignette, a higher percentage of patients wanted a shared role (44.3%) than with the other two vignettes. In the problem-solving component, preferences were significantly associated with area of residence (p<0.001) and educational level (p=0.013), while in the decision-making, component preferences were associated with age (p=0.020), educational level (p=0.015) and profession (p<0.001).ConclusionsIn this representative sample of the Portuguese mainland population, most patients preferred a practitioner-controlling role for both the problem-solving and decision-making components. In a life-threatening situation, patients were more willing to let the doctor decide. In contrast, in a less serious situation, there is a greater willingness to participate in decision-making. We have found that shared decision-making is more acceptable to better-educated patients in the problem-solving component and to people who are younger, higher educated and employed, in the decision-making component.

Background Although China has the largest population of persons with dementia, there is no validated tool available to accurately assess formal caregivers’ competence in dementia care in long-term care settings. Appropriately assessing nursing staff’s level of competence in dementia care is the first step to develop precision training interventions to improve the quality of dementia care. The Sense of Competence in Dementia Care Staff scale (SCIDS) is a user-friendly tool with satisfactory reliability and validity. We adapted SCIDS into a Chinese version (SCIDS-C) and validated its uses in China’s socio-cultural context to assess nursing staff’s capability and competence in dementia care at nursing homes. Aims We aimed to adapt and psychometrically test the tool among frontline nursing staff in long-term care settings in China. Methods The research employed a correlational design with repeated measures. In translation section, we adapted and tailored the original scale in the cultural and social context in China’s nursing homes. The scale’s adaptation consists of translating adaptation and semantic equivalence. In psychometric testing phase, we tested the validity and reliability of the scale with 174 nursing staff conveniently from six nursing homes. Construct validity was tested using exploratory factor analysis (EFA), including principal component analysis and maximum variance rotation method. Reliability was tested using Cronbach’s alpha value and intraclass correlation coefficient (ICC). Results The SCIDS-C has 17 items, which belong to the two sub-scales, the Relationship-Centered Care(RCC) and Professional Care(PC). The Cronbach’s alpha value was 0.88, showing a good internal consistency. The full scale’s value of ICC was 0.94 which indicated good reliability. Exploratory factor analysis(EFA) extracted 2 common factors in each sub-scale, cumulative variance contribution rate was 56.71% and 53.92%, respectively. The named four factors are the same as the Sense of Competence in Dementia Care Staff (SCIDS) scale in English, including Building Relationships, Sustaining Personhood, Professionalism and Care Challenges. Conclusion The SCIDS-C has shown good reliability and validity. It can be used as an appropriate tool to evaluate the competence of nursing care staff to provide dementia care for residents in nursing homes.

With the popularization of big data technology, intelligent translation technology gradually rises and is applied in daily life, and this paper explores its utility in English writing. English writing can benefit from the auxiliary utility of Big Data intelligent translation The grammatical error correction model for intelligent English translation is constructed based on statistical classification and deep classification to optimize the English translation system. Through the performance testing of the intelligent translation system, the performance of the algorithm model of this paper is improved by 3.03, 2.64, and 8.55 compared with CYK, CYK-PU, and the shift-and-return algorithm, based on which the intelligent translation system is applied to English writing for the evaluation and analysis of the effect, and the reliability coefficients of the three scales are greater than 0.9 through the reliability and validity test and correlation analysis. Finally, a practical analysis of the auxiliary utility of intelligent translation in English writing is conducted. Finally, the auxiliary utility of intelligent translation in English writing is practically analyzed, and in the final achievement test, its various ability test scores and comprehensive achievement scores are all greater than 0.9. An intelligent translation system can help cultivate and improve English writing ability, which enhances the realistic reference significance of intelligent translation to aid English writing.
