Bronchiolitis is a virus-associated infection of the lower respiratory tract exhibiting signs and symptoms of airway obstruction. Respiratory Syncytial Virus (RSV) is responsible in most cases; however, different rhinoviruses have also been implicated. Specific viruses and time until the first infection, severity of the respiratory condition, and atopic status have a determinant role in the recurrence of wheezing and asthma development. Genetics, lung function, atopic condition, the role of microbiota and environment, pollution, and obesity are considered in the present review. Emergency room visits and hospitalizations because of severe wheezing and smoking during pregnancy among others were identified as risk factors for significant morbidity in our population. Approaching determinant conditions like genetics, allergy, antiviral immunity, and environmental exposures such as farm vs. urban and viral virulence provides an opportunity to minimize morbidity of viral illness and asthma in children.

Introduction: Acute Respiratory Infection is an infection that occurs in the respiratory tract and is the main cause of death and mortality worldwide, especially in children aged 1-5 years. Air pollution from cigarette smoke is one of the substantial contributing factors to the incidence of ARI. It is estimated that 40-50% of children worldwide are regularly exposed to cigarette smoke, mainly from being around smoking parents. Toddlers who are exposed to cigarette smoke can get much higher substances of toxins and carcinogens than first-hand smoke. Children are more at risk for coughing, wheezing, excessive mucus production, and higher risk for various respiratory infections. Methods: This study used a literature review. of three international databases, there is Pubmed, EBSCO, and Sciencedirect with the keywords "Acute respiratory infection", " children under-five years", and "smoking behavior" that published between 2018-2022. The type of journal is the original and full text, and the design of this research is case-control and cross-sectional research. Result and discussion: There are 272 articles with the discovery of Pubmed 49 journals, EBSCO 72 journals, and Sciendirect is 151 journals. Then the researchers filtered through titles, research methods, and abstracts to find conformity with the relationship between smoke exposure from families of active smokers who have toddlers with the incidence of acute respiratory disease. A total of 17 articles can be employed in this study. Conclusion: It can be concluded that a smoking family can increase the incidence of Acute Respiratory Infection in under-five children.

Abstract. In Indonesia, acute respiratory infections (ARI) are still very common, especially in children under five, with a morbidity of 3% and a mortality of 15.5%. Many factors can cause this infection in toddlers, including smoking behavior in the family. The aim of this research was to study family smoking behavior among toddlers suffering from ISPA at the Ciasem Health Center. This study was designed as a quantitative retrospective. Toddler patients who came to the Ciasem Community Health Center in Subang Regency with complaints of ISPA were research subjects from June to September 2023. The sample was 41 toddlers. This study used consecutive sampling as a sampling technique. The family smoking behavior questionnaire was used as a research instrument. The data that has been collected will be analyzed using descriptive data analysis. The results of this study show that family smoking behavior among toddlers with ISPA at the Ciasem Community Health Center is in the moderate smoker category. The conclusion of this research is that the majority of families of toddlers with ISPA at the Ciasem Community Health Center have smoking behavior in the moderate category. Abstrak. Di Indonesia, infeksi saluran pernapasan akut (ISPA) masih sangat umum, terutama pada balita dengan morbiditas 3% dan mortalitas 15,5%. Banyak faktor dapat menyebabkan infeksi balita ini, termasuk perilaku merokok dalam keluarga. Tujuan penelitian ini adalah mempelajari perilaku merokok keluarga pada balita yang menderita ISPA di Puskesmas Ciasem. Penelitian ini dirancang sebagai kuantitatif retrospektif. Pasien balita yang datang ke Puskesmas Ciasem di Kabupaten Subang dengan keluhan ISPA adalah subjek penelitian dari Juni hingga September 2023. Sampel sebesar 41 balita. Penelitian ini menggunakan consecutive sampling sebagai teknik pengambilan sampel. Kuesioner perilaku merokok keluarga digunakan sebagai instrumen penelitian. Data yang telah terkumpul dianalisis menggunakan analisis data deskriptif. Hasil penelitian ini menunjukkan perilaku merokok keluarga pada balita dengan ISPA di Puskesmas Ciasem berada pada kategori perokok kategori sedang. Simpulan penelitian ini mayoritas keluarga balita dengan ISPA di Puskesmas Ciasem memiliki perilaku merokok dalam kategori sedang.
