Objectives: Rheumatoid arthritis (RA) is characterized by defective bone repair and excessive destruction and ankylosing spondylitis (AS) by increased ectopic bone formation with syndesmophytes. Since TNF-α and IL-17A are involved in both diseases, this study investigated their effects on the osteogenic differentiation of isolated human bone marrow-derived mesenchymal stem cells (hMSCs).Methods: Differentiation of hMSCs into osteoblasts was induced in the presence or absence of IL-17A and/or TNF-α. Matrix mineralization (MM) was evaluated by alizarin red staining and alkaline phosphatase (ALP) activity. mRNA expression was measured by qRT-PCR for bone morphogenetic protein (BMP)-2 and Runx2, genes associated with osteogenesis, DKK-1, a negative regulator of osteogenesis, Schnurri-3 and receptor activator of nuclear factor kappa B ligand (RANKL), associated with the cross talk with osteoclasts, and TNF-α receptor type I and TNF-α receptor type II (TNFRII).Results: TNF-α alone increased both MM and ALP activity. IL-17A alone increased ALP but not MM. Their combination was more potent. TNF-α alone increased BMP2 mRNA expression at 6 and 12 h. These levels decreased in combination with IL-17A at 6 h only. DKK-1 mRNA expression was inhibited by TNF-α and IL-17A either alone or combined. Supporting an imbalance toward osteoblastogenesis, RANKL expression was inhibited by TNF-α and IL-17A. However, TNF-α but not IL-17 alone decreased Runx2 mRNA expression at 6 h. In parallel, TNF-α but not IL-17 alone increased Schnurri-3 expression with a synergistic effect with their combination. This may be related to an increase of TNFRII overexpression.Conclusion: IL-17 increased the effects of TNF-α on bone matrix formation by hMSCs. However, IL-17 decreased the TNF-α-induced BMP2 inhibition. Synergistic interactions between TNF-α and IL-17 were seen for RANKL inhibition and Schnurri-3 induction. Such increase of Schnurri-3 may in turn activate osteoclasts leading to bone destruction as in RA. Conversely, in the absence of osteoclasts, this could promote ectopic bone formation as in AS.

Objective: To summarize domestic and foreign research on reparation and reconstruction progress for long bone segmental defects in recent years. Method: The first author applied computers to retrieve articles of PubMed database, Chinese Journal Full-text Database and VIP Database about long bone segmental defect reparation and reconstruction according to English search term; bone defect, long bone reconstruction, tissue engineering of bone, scaffolds, bone reconstruction, bone graft, bone tumor; tumor resection, musculoskeletal tumors, regeneration, autografts' and Chinese search term 'large segment, bone defect, bone tumor, soft tissue tumor, enbolc resetion, tissue engineering and bone graft' from January 1990 to December 2012. A total of 104 related articles were searched, and 52 related articles were included for further study. Results and conclusion: Traditional repair methods of long bone segmental defects include autogenous bone graft, allogeneic bone graft, bone lengthening, artificial prosthetic replacement, etc. Development of prosthesis suitable for children limb salvage demands and consistent with children growth characteristics becomes a challenge for children limb salvage. People develop prosthesis capable of constant lengthening in order to solve the problem, which is used in limb salvage reconstruction for children bone tumor, with progress of medical technology. Various artificial bones, especially gradually application of bone tissue engineering and gene treatment gene treatment in bone defect repair, will further improve the remediation effect of bone defects. It is future research direction.

Ankilozan spondilit (AS) yeni kemik oluşumu ile karakterize sistemik romatizmal bir hastalıktır. Spinal füzyon ile birlikte ilerleyici spinal fraktür riski artmaktadır. Ankilozan spondilitli spinal travmalı hastaların yönetimi sırasında çok dikkatli olunmalıdır. AS'li kaza yapmış hastada en sık karşılaşılan bulgu, lokal ağrıdır. Nonsteroid analjeziklere yanıt vermeyen travmalı ve AS'li hastalarda vertebra fraktürlerinden şüphelenmek gerekir. Omurga kırıkları genelde alt torakal ve üst lomber bölgede meydana gelir. Biz bu sunuda, ankilozan spondilitli spinal travmalı bir hastaya, uygulanan sıradışı enstrümantasyon işlemini literatür eşliğinde sunmayı amaçladık.Ankylosing spondylitis is a systemic rheumatic disease characterized by the formation of new bone. Increases the risk of progressive spinal fracture with spinal fusion. Patients who spinal trauma with ankylosing spondylitis during management must be taken. With a motor vehicle accident in the most common symptoms of ankylosing spondylitis patients, is local pain. Nonsteroidal analgesic do not respond to trauma and vertebral fractures in patients with ankylosing spondylitis should be suspected of. In this case, we aim which have applied an unusual instrumentation in a patients with trauma spinal vertebrae with ankylosing spondylitis Anahtar Kelimeler: Ankilozan spondilit, torakolomber travma, posterior enstrümantasyon.
