Purpose: Patients who have suffered an ocular trauma may present with varying degrees of injury to the anterior segment. In this retrospective interventional case series, we report the outcome of seven patients who underwent complete anterior segment reconstruction in a single surgery. Methods: All patients with posttraumatic corneal decompensation or scar, aphakia, and iris defect underwent human donor corneal graft transplantation and implantation of an intraocular lens combined with a flexible silicone iris prosthesis. Postoperative examinations included assessment of best corrected distance visual acuity, objective refraction, and intraocular pressure. Sensitivity to glare and subjective discontent with the eye’s appearance was rated on a scale from 1 to 10, with 1 standing for low and 10 for high severity. Results: Mean best corrected distance visual acuity (BCDVA) was 1.51 ± 0.26 logMAR preoperatively and 1.29 ± 0.36 logMAR postoperatively. Mean IOP was 15.71 ± 8.94 mmHg pre-surgery and 13.57 ± 6.52 mmHg post-surgery. The mean sensitivity to glare was reduced from 7.17 ± 2.91 to 3.80 ± 3.43 and subjective cosmetic disfigurement was reduced from 5.33 ± 3.35 to 1.80 ± 1.60. Conclusions: A single surgery technique for entire anterior segment reconstruction in trauma patients can effectively reduce glare and patient discontent with the eye’s appearance.

A family of poly--lysine hydrogels can be synthesized by crosslinking with bis-carboxylic acids using carbodiimide chemistry. In addition to creating hydrogels using a simple cast method, a fragmented method is used to introduce increased porosity within the hydrogel structure. Both methods have created tunable characteristics ranging in their mechanical properties, transparency, and water content, which is of interest to corneal tissue engineering and can be tailored to specific cellular needs and applications. With a worldwide shortage of cornea donor tissue available for transplant and limitations including rejection and potential infection, a synthetic material that can be used as a graft, or a partial thickness corneal replacement, would be an advantageous treatment method. These hydrogels can be tuned to have similar mechanical and transparency properties to the human cornea. They also support the attachment and growth of corneal epithelial cells and the integration of corneal stromal cells.

The lack of endothelial layer hinders the use of decellularized corneal stroma in keratoplasty, resulting in adverse effects, such as non‐specific protein adsorption and corneal oedema after implantation, which leads to rapid failure of the ophthalmic implants. In this study, superhydrophilic poly(2‐methacryloyloxyethyl phosphorylcholine) (PMPC) was gently introduced to the porcine‐derived decellularized corneal stroma matrix (pDCSM), aiming to resist undesirable biofilm adsorption within the ocular environment. After complete decellularization, the pDCSM was first methacrylated by the integration of methacrylic anhydride. Consecutively, PMPC was only grafted from the back surface (endothelium side) of the methacrylated pDCSM through surface‐initiated free radical polymerization. This one‐side surface‐modified pDCSM not only retained good optical transmittance and mechanical properties that were comparable to the untreated pDCSM, but both surfaces of the same artificial cornea also showed non‐cytotoxicity and good biocompatibility. Moreover, the PMPC‐grafted back surface exhibited considerable antifouling properties that resisted both protein and cell adhesion. Consequently, such Janus‐like artificial cornea holds great promise in future ophthalmic applications, which may serve as a springboard for the design of versatile decellularized extracellular matrix based biomedical implants with Janus‐like properties.
