The Role of Vitreoretinal Surgery in the Treatment of Posttraumatic Macular Hole

Abstract: Our results suggest that intraoperative application of platelet concentrate in combination with vitrectomy may be useful in managing posttraumatic full-thickness macular holes, thus improving anatomic and visual outcomes. The greater recovery of visual acuity obtained in this study compared with that obtained in other series of idiopathic macular holes could be related to the young age of the patients with traumatic macular holes and the early diagnosis and treatment.

“…indirect viewing system that provides the surgeon with a wide field of view. 1 The image, although of sufficiently wide field, is inverted, which makes manipulation of instruments inside the eye hazardous. Reinverting the image has been achieved by incorporating a prism system into the microscope 2 that has undergone refinement and modification.…”

“…The results of surgery for traumatic macular hole closure are favorable. 119,120 The hyaloideoretinopathies include a number of diseases with much overlap in the clinical and sometimes genetic characteristics. Most of these diseases share common dystrophic retinal and vitreal changes; however, they sometimes differ by the predominance of one of these changes.…”

“…In 1995, some authors (5) described a 92% surgical closure rate and a final visual acuity of 20/40 or better in 12 eyes with TMH. Two years later, another group described a series of 14 cases where a 93% closure rate was observed after surgery and vision improved at least four Snellen lines in every closed hole (7) . In 1999, a retrospective case series of 16 eyes with TMH revealed a 94% closure rate and a vision of 20/40 or better in 38% of the eyes (9) .…”

“…Besides the mechanism of TMH formation, the decision of whether to operate or simply observe these TMH is also controversial. Several retrospective case series have shown positive outcomes of pars plana vitrectomy for TMH (3,(5)(6)(7)(8)(9)(10)(11) . On the other hand, many cases of spontaneous closure of TMH have been reported in the literature (12)(13)(14)(15)(16)(17)(18)(19) .…”
