The roles of comorbidity and trauma exposure and its timing in shaping HPA axis patterns in depression

Abstract: Background: Much work has documented hypothalamic pituitary adrenal (HPA) axis abnormalities in major depressive disorder (MDD), but inconsistencies leave this system's role in the illness unclear. Comparisons across studies are complicated by variation in co-morbidity (Posttraumatic Stress Disorder-PTSD, anxiety disorders), exposure to trauma, and timing of trauma (child vs. adult). Here, we examined the impact of these factors on HPA profiles in depression. Methods:We recruited 5 groups of participants: MDD … Show more

“…Although no differences were demonstrated in long term hair cortisol assays between those with childhood trauma vs. those without, patients who were unresponsive to treatment revealed lower cortisol levels prior to psychological treatment (154). A further study investigating differences in baseline cortisol in patients with comorbid psychiatric illnesses found no baseline cortisol differences in either children or adults with comorbid MDD and PTSD (155).…”

“…Depression complicated with anxiety, a specific subtype of depression, is associated with poorer clinical outcomes, such as worse psychosocial functioning and treatment response. Therefore, uncovering the neurobiology of this subtype is particularly important for improving diagnosis, prognosis, and treatment ( 31 ). This work also found that BMI, SBP, and triglyceride in patients with depression complicated with anxiety were significantly higher than those in patients with depression.…”

“…Patients with MDD with childhood trauma (CT) have a low baseline cortisol level, together with reduced ACTH levels at the baseline and Trier Social Stress Test ( Mayer et al, 2020 ). PS causes a higher basic level of ACTH in animals ( Lolait et al, 2007 ).…”
