The significance of allergic contact urticaria to milk in children with cow's milk allergy

Abstract: CMP contact urticaria exists only in patients with IgE-mediated CMA. A 'finger test' to CMP should be part of the evaluation of CMA patients, and positivity suggests the potential for multiple food allergies, especially to sesame and egg.

“…This so-called "finger test" could provide additional confirmation of CMA. 11 Contrary to the results of other studies that showed no significant specificity of abdominal colic, 12e15 we demonstrated this clinical sign to be the most specific for CMA. Although the etiology of abdominal colic is multifactorial, some studies have shown that even without confirmation of CMA, the introduction of partially or extensively hydrolyzed CM leads to improvement in the clinical picture, probably because of the favorable effect of hydrolyzed formula on intestinal motility and digestion.…”

“…Also contact urticaria is typically associated only with IgE-mediated CMA. 86 Upon ingestion of the milk, the smooth muscles in esophagus and stomach can quickly react and cause vomiting, or in the intestine lead to diarrhea (immediate gastrointestinal hypersensitivity). Absorption of milk proteins via the mucosa can occur at any of these time points and potentially lead to systemic reactions, like skin exacerbations (acute urticaria, erythema, angioedema), asthma attacks, and also anaphylactic shock in severely milk allergic patients.…”
