PurposeProstaglandin E2 (PGE2) is a contributory carcinogen in gastric adenocarcinoma. 15-Hydroxyprostaglandin dehydrogenase (15-PGDH) catabolizes PGE2 by oxidizing its 15(s)-hydroxy group. The aim of this study was to investigate the expression of 15-PGDH in gastric adenocarcinoma tissue and the relationship between 15-PGDH expression and clinicopathologic features of gastric adenocarcinoma.MethodsNinety-nine patients who underwent surgical resection for gastric adenocarcinoma between January 2007 and December 2007 were enrolled and evaluated retrospectively.ResultsIn 62 patients (62.6%), 15-PGDH expression was lower in gastric adenocarcinoma tissue than in nonneoplastic tissue. Regarding the relationship between 15-PGDH expression and clinicopathological features, 15-PGDH expression was significantly lower in tissues with poor differentiation (P = 0.002), advanced T stage (P = 0.0319), a higher number of lymph node metastases (P = 0.045), lymphatic invasion (P = 0.031), and vascular invasion (P = 0.036).Conclusion15-PGDH expression was associated with a subset of clinicopathologic features such as differentiation grade, T stage, lymphatic invasion, and vascular invasion.

Purpose:Recently, early gastric cancer has increased in Korea. Thus, endoscopic treatment and laparoscopic gastrectomy has increased in early gastric cancer patients. We studied periodic change and characteristics in gastric cancer patients. Thus, we analyzed annual change of clinicopathological characteristics and long-term survival results of gastric cancer patients after radical gastrectomy over 10 years. Methods: From 1995 to 2004, 2,387 patients underwent radical gastrectomy due to gastric cancer. We analyzed annual characteristics, sex, age distribution, cancer location, depth of invasion, lymph node metastasis, UICC stage and 5-year survival rates, retrospectively from medical records. Results: The number of gastric cancer patient has increased annually. The ratio of male to female was 2：1.Gastric cancer was most common and increased annually in the 60∼79-year age group. Early gastric cancer increased annually. The 5-year survival rate was 93% in stage Ia, 89% in stage Ib, 72% in stage II, 51% in stage IIIA, 38% in stage IIIb and 22% in stage IV. Multivariate analysis revealed that UICC stage (or depth of invasion and lymph node metastasis) and cancer location were the main independent prognostic factors. Conclusion: Prognosis is improved due to increase of early gastric cancer. So, we should attempt diagnosis early and treat early gastric cancer. Active treatment is recommended even for the elderly gastric cancer patients. And care should be taken with respect to morbidity and mortality. (J Korean Surg Soc 2009;77:88-95)

PurposeThe incidence of lymph node metastasis has been reported to range from 2.6 to 4.8% in early stage gastric cancer with mucosal invasion (T1a cancer). Lymph node metastasis in early stage gastric cancer is known as an important predictive factor. We analyzed the prediction factors of lymph node metastasis in T1a cancer.Materials and MethodsA total of 9,912 patients underwent radical gastrectomy due to gastric cancer from October 1994 to July 2006 in the Department Of Surgery at Samsung Medical Center. We did a retrospective analysis of 2,524 patients of these patients, ones for whom the cancer was confined within the mucosa.ResultsAmong the 2,524 patients, 57 (2.2%) were diagnosed with lymph node metastasis, and of these, cancer staging was as follows: 41 were N1, 8 were N2, and 8 were N3a. Univariate analysis of clinicopathological factors showed that the following factors were significant predictors of metastasis: tumor size larger than 4 cm, the presence of middle and lower stomach cancer, poorly differentiated adenocarcinoma and signet-ring cell carcinoma, diffuse type cancer (by the Lauren classification), and lymphatic invasion. Multivariate analysis showed that lymphatic invasion and tumor larger than 4 cm were significant factors with P<0.001 and P=0.024, respectively.ConclusionsThe frequency of lymph node metastasis is extremely low in early gastric cancer with mucosal invasion. However, when lymphatic invasion is present or the tumor is larger than 4 cm, there is a greater likelihood of lymph node metastasis. In such cases, surgical treatments should be done to prevent disease recurrence.
