Aim To assess deintensification approaches and rates and evaluate the harm and benefits of deintensification with antidiabetic medication and other therapies among older people (≥ 65 years) with type 2 diabetes with or without cardiometabolic conditions. Methods We identified relevant studies in a literature search of MEDLINE, Embase, Web of Science and Cochrane databases to 30 October 2018. Data were extracted on baseline characteristics, details on deintensification and outcomes, and was synthesized using a narrative approach. Results Ten studies (observational cohorts and interventional studies) with data on 26 558 patients with comorbidities were eligible. Deintensification approaches included complete withdrawal, discontinuation, reducing dosage, conversion, or substitution of at least one medication, but the majority of studies were based on complete withdrawal or discontinuation of antihyperglycaemic medication. Rates of deintensification approaches ranged from 13.4%–75%. The majority of studies reported no deterioration in HbA1c levels, hypoglycaemic episodes, falls or hospitalizations on deintensification. On adverse events and mortality, no significant differences were observed among the comparison groups in the majority of studies. Conclusion Available but limited evidence suggests that the benefits of deintensification outweigh the harm in older people with type 2 diabetes with or without comorbidities. Given the heterogeneity of patients with diabetes, further research is warranted on which deintensification approaches are appropriate and beneficial for each specific patient population.

Диагностика, контроль, лечение Diagnosis, control, treatment тремительное старение населения во всем мире представляет собой одно из самых значимых демографических, социально-экономиче-ских и политических преобразований нашего времени. По данным ООН, доля лиц в возрасте 60 лет и старше удвоится в период между 2007 и 2050 гг., а их реальное число увеличится более чем в три раза, до-стигнув 2 млрд в 2050 г. [1]. Данная проблема требует коренных изменений в системе здравоохранения для предоставления комплексной помощи, учитывающей особые потребности пожилых людей. В 2015 г. Всемир-ной Организацией Здравоохранения опубликован Все-мирный доклад о старении и здоровье, в котором авторы подчеркивают крайнюю актуальность данной проблемы и рекомендуют массовые исследования пожилых людей для выявления уровня функциональной способности, конкретного состояния здоровья, потребности в меди-цинской помощи, уходе и поддержке для поддержания «здорового старения» [2].Рост распространенности сахарного диабета 2 типа (СД2) является следствием старения населения. Со-гласно данным International Diabetes Federation (IDF), 415 млн взрослых людей в возрасте 20-79 страдают диа-бетом во всем мире, а распространенность диабета у лиц старше 65 лет достигает 20% [3]. В России в настоящее время, по данным Государственного регистра больных СД, на январь 2015 г. зарегистрировано 4,095 млн боль-ных СД, что составляет 2,8% населения РФ. За 5-летний период рост распространенности СД в РФ составил 23% (930 тыс. пациентов) [4]. По данным российских иссле-дователей, распространенность СД2 возрастала после 50 лет от 3,0% в возрастной группе 41-50 лет до 10,6% в возрастной группе 51-60 лет, достигая максимума у лиц старше 70 лет -12,6% [5].Согласно классификации, принятой европейским региональным бюро ВОЗ в 1963 г., пожилым считается возраст от 60 до 74 лет, старческим -от 75 до 89 лет, а возрастом долгожителей -90 лет и более. До настоя-щего времени эта классификация не пересмотрена. В большинстве американских и европейских исследо-ваний критерием пожилого возраста является возраст 65 лет и старше, однако рассматриваются и другие воз-растные группы населения, такие как 50 лет и старше, 60 лет и старше и 80 лет и старше. The social burden of hypoglycemia in the elderly

Abstract: The occurrence of symptoms of hypoglycemia is very common among older adults with diabetes and their presence is associated with an extremely negative impact on quality of life. Minimizing the risk of hypoglycemia represents a high priority in the diabetes treatment of elderly people.
