Background: Double burden of malnutrition (DBM) is an emerging public health concern in urban Benin. However, an understanding of the phenomena in rural areas of the country is lacking. Objectives: To assess the prevalence of DBM and to investigate the sociodemographic and dietary characteristics that differentiate DBM from undernourished children only, overweight/obese mothers only, and normal households in Kalalé district. Methods: A cross-sectional study was conducted on 426 mother-child pairs: nonpregnant women aged 15 to 49 years and children aged 6 to 59 months. Weight-forage and height-forage were used to classify underweight and stunted children, body mass index !25 kg/m 2 to measure overweight/obese mothers, and a 24-hour recall to assess the diet diversity score (DDS). Results: Overall, 37.6% of children were stunted, 10.1% wasted, and 22.8% underweight, while the rate of overweight/obese mothers was 15.5%. The DBM was present in 6.1% of the households. Overweight/obese mothers' households had better socioeconomic status (SES), greater maternal education, less food insecurity, and a more diversified diet, in contrast with undernourished children households. The DBM households shared several features with undernourished children households, except for a greater (not significant) SES, but had the lowest DDS. Logistical regression revealed that high SES, older age child, DDS, mother education, and ethnicity were associated with DBM. Conclusions: The study highlights the importance of addressing the DBM at the community level in rural Benin. However, multicenter studies in various rural parts of the country are needed to substantiate the present results, so that appropriate strategies to reduce the DBM can be planned.

Although the prevalence of undernutrition among women of reproductive age has declined in Bangladesh, the increase in the prevalence of overnutrition remains a major challenge. To achieve Sustainable Development Goal 2.2, it is important to identify the drivers of the double burden of malnutrition on women in Bangladesh. The Bangladesh Demographic and Health Survey, 2017–2018 was used to model the relationship between the double burden of malnutrition among women and the risk factors using a logistic regression model under the classical and Bayesian frameworks and performed the comparison between the regression models based on the narrowest confidence interval. Regarding the Bayesian application, the Metropolis‐Hastings algorithm with two types of prior information (historical and noninformative prior) was used to simulate parameter estimates from the posterior distributions. The Boruta algorithm was used to determine the significant predictors. Almost half of reproductive aged women experienced a form of malnutrition (12% were underweight, 26.1% were overweight, and 6.8% were obese). In terms of the narrowest interval estimate, it was found that Bayesian logistic regression with informative priors performs better than the noninformative priors and the classical logistic regression model. Women who were older, highly educated, from rich families, unemployed, and from urban residences were more likely to experience the double burden of malnutrition. This study recommended using the historical prior as the informative prior rather than the flat/noninformative prior to estimating the parameter uncertainty if historical data are available. The double burden of malnutrition among women is a major public health challenge in Bangladesh. This study was to determine the impact of effective risk factors on the double burden of malnutrition among women by applying the Bayesian framework. Using both informative and noninformative priors, “historical prior” was proposed as informative prior information. The main strength is that the proposed prior (historical prior) provided improved estimation as compared to the flat prior distribution.

BackgroundThe WHO recommended criteria for diagnosis of sever acute malnutrition (SAM) are weight-for-height/length Z-score (WHZ) of <− 3Z of the WHO2006 standards, a mid-upper-arm circumference (MUAC) of < 115 mm, nutritional oedema or any combination of these parameters. A move to eliminate WHZ as a diagnostic criterion has been made on the assertion that children with a low WHZ are healthy, that MUAC is a “superior” prognostic indicator of mortality and that adding WHZ to the assessment does not improve the prediction of death. Our objective was to examine the literature comparing the risk of death of SAM children admitted by WHZ or MUAC criteria.MethodsWe conducted a systematic search for reports which examined the relationship of WHZ and MUAC to mortality for children less than 60 months. The WHZ, MUAC, outcome and programmatic variables were abstracted from the reports and examined. Individual study’s case fatality rates were compared by chi-squared analysis and random effects meta-analyses for combined data.ResultsTwenty-one datasets were reviewed. All the patient studies had an ascertainment bias. Most were inadequate because they had insufficient deaths, used obsolete standards, combined oedematous and non-oedematous subjects, did not report the proportion of children with both deficits or the deaths occurred remotely after anthropometry. The meta-analyses showed that the mortality risks for children who have SAM by MUAC < 115 mm only and those with SAM by WHZ < −3Z only are not different.ConclusionsAs the diagnostic criteria identify different children, this analysis does not support the abandonment of WHZ as an important independent diagnostic criterion for the diagnosis of SAM.
