The spectrum of nervous system involvement in Behçet’s syndrome and its differential diagnosis

Abstract: Behçet's Syndrome (BS) is a multi-system, vascular-inflammatory disease of unknown origin, involving the nervous system in a subgroup of patients. The growing clinical and imaging evidence suggests that primary neurological involvement in BS may be subclassified into two major forms: the first one, which is seen in the majority of patients, may be characterized as a vascular-inflammatory central nervous system (CNS) disease, with focal or multifocal parenchymal involvement mostly presenting with a subacute bra… Show more

“…The 2 types rarely occur in the same individual, and their pathogeneses are probably different. 5,6 The precise pathologic mechanism of parenchymal NBD lesions has not been established. Parenchymal NBD lesions have been hypothesized to be venous infarcts 7 ; however, this hypothesis needs more pathologic support because vasculitis cannot usually be demonstrated within parenchymal NBD lesions.…”

“…The CNS manifestations of the disease are protean and include meningoencephalitis, cranial neuropathies, cerebrovenous sinus thrombosis, seizures, movement disorders, ataxia, cognitive dysfunction, and psychosis (Siva and Saip, 2009). Myelitis is a rare complication of Behçet's disease.…”

“…The brainstem is commonly involved, but other brain regions, as well as the spinal cord, may also be involved [169][170][171]. Aseptic meningitis also occurs [172]. In addition to inflammatory pathology in the brain, patients with Behçet's are prone to the development of cerebral vein thrombosis (CVT).…”
