The Surgical Management of Kidney Stone Disease: A Population Based Time Series Analysis

Abstract: Our population based study demonstrates a shift in the treatment paradigm with increased use of ureteroscopy over time and a reciprocal decrease in the use of extracorporeal shock wave lithotripsy. We also observed a corresponding decrease in ancillary treatment and increase in posttreatment morbidity over time.

“…The table lists demographic characteristics of the 116,115 patients with a mean age of 50.8 years (range 18 to 101) who comprised the OKSC. 15 The mean age of patients treated with SWL, URS and PCNL was 50.4 (range 18 to 96), 50.8 (range 18 to 101) and 54.6 years (range 18 to 94), respectively. During the study period the rate of kidney stone procedures performed per year increased steadily by approximately 48% from 85/100,000 to 126/100,000 population.…”

“…After exclusions we identified a total of 194,781 procedures performed in 116,115 patients between July 1, 1991 and December 31, 2010, representing the study cohort previously defined as OKSC, 15 for our cross-sectional time series analysis. The table lists demographic characteristics of the 116,115 patients with a mean age of 50.8 years (range 18 to 101) who comprised the OKSC.…”

“…4 It is not surprising, then, that surgical techniques such as ureteroscopy and percutaneous nephrolithotomy (PCNL), procedures which both commonly utilize the HoYAG laser, are being increasingly performed relative to shockwave lithotripsy for the management of kidney stones. 5,6 Current practice guidelines advocate PCNL as the preferred treatment for large renal stones. 7,8 Traditional methods of lithotripsy during PCNL have relied upon ultrasonic, ballistic, or a combination of these energy delivery devices.…”
