The ‘Top 10’ Challenges for Health Technology Assessment: INAHTA Viewpoint

Abstract: The International Network of Agencies for Health Technology Assessment (INAHTA) spans the globe as a network of 50 publicly-funded health technology assessment (HTA) agencies supporting health system decision making for 1.4 billion people in thirty countries. Agency members are non-profit HTA organizations that are part of, or directly support, regional or national governments. Recently, INAHTA surveyed its members to gather perspectives from agency leadership on the most important issues in HTA today. This pa… Show more

“…Our findings indicate that there are only a limited number of robust and complete HTA reports for biosimilars, mainly due to the absence of a systematic literature review, failures in the appraisal of the quality of the evidence, and a lack of consideration of economic issues such as budget impact and cost-effectiveness. A summary of the top 10 challenges to produce an HTA identified by INAHTA members pointed out that the increasing demand for HTAs was often accompanied by a request for greater speed, leading to an increased demand for rapid HTA [17]. Additionally, INAHTA members considered that the use of an evidence grading system when formulating recommendations was a challenge [17].…”

“…A summary of the top 10 challenges to produce an HTA identified by INAHTA members pointed out that the increasing demand for HTAs was often accompanied by a request for greater speed, leading to an increased demand for rapid HTA [17]. Additionally, INAHTA members considered that the use of an evidence grading system when formulating recommendations was a challenge [17]. This is reflected in the findings of this scoping review, since the majority of reports did not adequately appraise the quality of evidence with an appropriate approach or tools [38] such as the GRADE system for assessment of overall quality of evidence or tools to assess the methodological quality of primary studies such as the Assessing the Methodological Quality of Systematic Review (AMSTAR) and the Cochrane Risk of Bias tools.…”

“…HTA bodies require information and expertise from a range of stakeholders to develop guidance (8). One key challenge is designing meaningful approaches to involving stakeholders and patients (9), ensuring that PPI is not tokenistic but adds value. Effective PPI approaches cannot be developed by HTA bodies in isolation; instead, they should be coproduced with stakeholders.…”
