Light ion beam treatments are becoming more widely used. Safe and optimal treatments may only be achieved when uncertainties are considered at every step of the planning and delivery process. These uncertainties include, but are not limited to, penetration uncertainties due to beam delivery, uncertainties in dose compliance, uncertainties of x-ray computed tomography numbers, absolute and relative linear stopping powers, absolute and relative linear scattering powers, conversion of x-ray computed tomography numbers to relative linear stopping power, lateral alignment uncertainties, and uncertainties due to inter-fractional and intra-fractional anatomical variations. Knowing the source and magnitude of these uncertainties, the planner must optimize the plans to mitigate the effect of these uncertainties as much as possible without making the plan undeliverable. Visualization of dose distributions considering the effects of these uncertainties is an important step in evaluating the safety and effectiveness of the plans. This report by Task Group 202 of the AAPM has endeavored to address each of these topics as a guide to the user of light ion beam treatments.

For an optimal exploitation of the benefits of proton therapy the most accurate dose delivery system should be used. The TERA Foundation has extensive experience in the field of high gradient high frequency linacs. This paper describes a particular design of a 3 GHz linac boosting the typical cyclotron beams for proton therapy of 230-250 MeV up to 350 MeV. Such an upgrade of a typical proton therapy facility enables performing proton radiography, as well as extending therapeutic capabilities with high energy proton therapy (HEPT). The recent studies and measurements in high-gradient linac technology demonstrated that average fields in the accelerating structures of up to 25-30 MV=m can be achieved, which results in a total linac length of less than 7 m. To test several characteristics of such a linac as a booster of a cyclotron beam, a design has been made of a linac unit accelerating from 250 MeV to 275 MeV, which could be built and inserted for tests in an existing beam line at the PSI proton therapy facility. The feasibility considerations, along with the design of the linac booster and the issues related to a possible integration in an existing cyclotron beam line are detailed in this study.

The Italian National Center for Hadrontherapy (CNAO, Centro Nazionale di Adroterapia Oncologica), a synchrotron‐based hospital facility, started the treatment of patients within selected clinical trials in late 2011 and 2012 with actively scanned proton and carbon ion beams, respectively. The activation of a new clinical protocol for the irradiation of uveal melanoma using the existing general‐purpose proton beamline is foreseen for late 2014. Beam characteristics and patient treatment setup need to be tuned to meet the specific requirements for such a type of treatment technique. The aim of this study is to optimize the CNAO transport beamline by adding passive components and minimizing air gap to achieve the optimal conditions for ocular tumor irradiation. The CNAO setup with the active and passive components along the transport beamline, as well as a human eye‐modeled detector also including a realistic target volume, were simulated using the Monte Carlo Geant4 toolkit. The strong reduction of the air gap between the nozzle and patient skin, as well as the insertion of a range shifter plus a patient‐specific brass collimator at a short distance from the eye, were found to be effective tools to be implemented. In perspective, this simulation toolkit could also be used as a benchmark for future developments and testing purposes on commercial treatment planning systems.PACS numbers: 21.30Fe, 24.10.Lx, 29.20.dk, 29.27.Eg, 29.85.Fj
