The world health organization's histological classification of salivary gland tumors. A commentary on the second edition

Abstract: The second edition of the World Health Organization's Histological Classification of Salivary Gland Tumors is more extensive and detailed than the previous edition published 20 years ago. The new edition is based on data regarding newly described tumor entities and the behavior and prognosis of the previously classified tumors. The distinct morphologic features of monomorphic adenomas justify their separation for purposes of identification. Among the carcinomas, various types were distinguished for purposes of… Show more

“…These findings are similar to other publications 12 , although they are different from studies reporting the mucoepidermoid carcinoma as the most frequent malignant neoplasia 2,14 . However, in the present study, two cases represent the difference between the number of cystic adenoid carcinoma and of mucoepidermoid, which is not considered significant when compared to other series.…”

“…Adjusted for each other, the ln (hazard ratios) are 0.73 (Ϯ 0.15) for PS2, 0.93 (Ϯ 0.35) for spill, and 1.01 (Ϯ 0.49) for clinical grade of malignancy according to the 1991 WHO classification. 9,10 Testing the proportional hazards assumption, no indication for a changing prognostic value of PS1 in time was found. We found no evidence of nonlinearity of the relation between PS1 and ln (hazard) (P ϭ 0.36).…”

“…We are not inclined to include in the model the variables tumor spill and clinical grade of malignancy according to the 1991 WHO classification. 9 We believe that the multiple comparisons phenomenon may well be responsible for the apparent significance of these variables. The multiple comparisons-corrected P values accompanying their addition were only 0.073 (9 ϫ 0.0081) and 0.387 (9 ϫ 0.043).…”

“…[1][2][3] Local and distant metastases are common features of this entity, regardless of the histological features or the status of surgical margins. 4,5 Alterations of cell adhesion molecules or production of excessive matrix protein have been proposed as factors associated with the aggressive behavior in adenoid cystic carcinoma. 6,7 However, the underlying events for the aggressive nature and protracted course of this tumor are poorly understood.…”
