Therapeutic drug monitoring of newer antiepileptic drugs / Therapeutic drug monitoring bei neueren Antiepileptika

Abstract: Therapeutic drug monitoring (TDM) of antiepileptic drugs (AEDs) has been in clinical use for more than three decades. Its value has, however, been questioned recently for several reasons, e.g., the occurrence of newer AEDs, for which the use of TDM has to be evaluated, but also because of fi nancial strains on health systems. The current review aims at assessing the use of TDM in newer AEDs, i.e., those that entered the market since the beginning of the 1990s. As some have been licensed only recently and other… Show more

“…1 The value of TDM is critically discussed from time to time, but TDM does have several indications in the assessment of older and newer AEDs. 2 It is usually determined at trough levels from blood serum or plasma before intake of the morning dose. 3 Blood serum is the favored matrix for analysis because this material is homogeneous and does not clot.…”

“…Therapeutic drug monitoring (TDM) plays an important role in the planning and evaluation of therapeutic strategies in epileptology. 1 Venous blood collection is an established method for obtaining specimens for TDM; however, it is time consuming and may be distressing for several groups of patients, such as children or persons with intellectual disability. Therefore, alternative methods must be established.…”

“…Furthermore, placebocontrolled clinical studies report the efficacy of GAB for the treatment of menopausal hot flashes (Guttuso et al, 2003;Butt et al, 2008) and restless legs syndrome (Garcia-Borreguero et al, 2002;Lee et al, 2011;Lal et al, 2012). The indication of therapeutic drug monitoring (TDM) has been suggested for GAB in epilepsy (Neels et al, 2004;Brandt, May, 2011;Krasowski, McMillin, 2014;Landmark et al, 2015;Patsalos, Spencer, Berry, 2018;Reimers et al, 2018). However, the benefits and therapeutic ranges for TDM of GAB in other indications have been poorly investigated so far (Burns et al, 2019).…”
