Three-year outcome of the covered endovascular reconstruction of the aortic bifurcation technique for aortoiliac occlusive disease

Abstract: The CERAB technique is a safe and feasible technique for the treatment of extensive AIOD with good 3-year results regarding patency and clinical improvement.

“…When we compare these data with the previously mentioned individual-participant meta-analysis on the treatment of kissing stents, it seems that the CERAB group included more complex patients with more critical limb ischemia, more TASC-C and TASC-D lesions and more previously performed reinterventions (Table 2). 6,18 Yet at 24-months, the primary and secondary patency rates were better in the CERAB cohort, suggesting that indeed with CERAB better results could be achieved. A prospective comparative trial on this subject, however, is warranted.…”

“…In 2018, Taeymans et al described the mid-term outcome in a series of 130 consecutively electively treated CERAB patients, including the first in human. 18 This was an update from the earlier published one-year data on a smaller cohort. 12 Chimney procedures were excluded from this analysis.…”

“…The covered endovascular reconstruction of the aortic bifurcation technique uses an endovascular approach to place three covered stents in the aortic bifurcation. This technique has demonstrated short-term superiority over the traditional aortobifemoral grafts, highlighting that the first one allows the treatment of patients with more comorbidities and with more complex stenosis 4. The survival rate of patients with aortoiliac occlusive disease is determined by the surgical success, the long-term permeability of the graft, the control of risk factors and cessation of tobacco consumption, among others 5…”

“…In recent years, there has been a marked demographic shift with increasing prevalence of diabetes among patients with SLI and intervention in older, frailer patients 11 38. While there has been little change in recommended medical therapy of PAOD,4 39 new endovascular technologies and improved techniques such as ultradistal bypass and covered endovascular revascularisation of aortic bifurcation allow increasingly aggressive revascularisation strategies 40–42. The Leicester Vascular Institute (based at Glenfield Hospital, University Hospitals of Leicester NHS Trust) is a tertiary referral vascular unit providing all vascular services for Leicestershire and Rutland, compromising a diverse population of approximately 1.1 million.…”
