Timing of removal of sutures in control of post-operative astigmatism

Abstract: A prospective, randomised study was carried out in 135 patients to determine whether the time of removal of continuous sutures in the relief of post-operative astigmatism had any effect on subsequent changes in cylinder power or axis. After uncomplicated extracapsular cataract extraction with a corneal section and continuous 10/0 nylon suture, patients with more than 3 dioptres of cylinder were allocated to have their suture removed at 6, 9 or 12 weeks post-operatively. Visual and optical outcome were assessed… Show more

“…7 In adults, suture removal is recommended to correct postoperative astigmatism. 8,9 In contrast, in a previous study, 10 we report that in children, high early postoperative astigmatism after congenital cataract surgery spontaneously decreased during the first months after surgery and that therefore, the indication for suture removal in these …”

“…This shifting phenomenon has previously been described in adults. 10 Using retinoscopy as a method for measuring astigmatism, resulted in the measurement of both corneal and lenticular astigmatism.…”

“…[4][5] An effective way to reduce or eliminate the post-operative astigmatism is by the removal of one or more interrupted or continuous sutures. [6][7][8][9] Removal of sutures was recommended only in eyes with astigmatism of at least 3.0 D. 7,10 Suture removal relieves wound compression, thereby altering the corneal curvature. In this study we describe the changes in astigmatism that occurred without suture cutting after congenital cataract surgery and intraocular lens (IOL) implantation.…”
