Summary:Purpose: Management of seizures in learning disabled people is challenging. This prospective study explored the efficacy and tolerability of adjunctive topiramate (TPM) in patients with learning disability and refractory epilepsy attending a single centre.Methods: Sixty-four patients (36 men, 28 women, aged 16-65 years) were begun on adjunctive TPM after a 3-month prospective baseline on unchanged medication. Efficacy end points were reached when a consistent response was achieved over a 6-month period at optimal TPM dosing. These were seizure freedom or Ն50% seizure reduction (responder). Appetite, behaviour, alertness, and sleep were assessed by caregivers throughout the study.Results: Sixteen (25%) patients became seizure free with adjunctive TPM. There were 29 (45%) responders. A further 10 (16%) patients experiencing a more modest improvement in seizure control continued on treatment at the behest of their family and/or caregivers. TPM was discontinued in the remaining nine (14%) patients, mainly because of side effects. Final TPM doses and plasma concentrations varied widely among the efficacy outcome groups. Many patients responding well to adjunctive TPM did so on Յ200 mg daily. Mean carer scores did not worsen with TPM therapy. Conclusions: TPM was effective as add-on therapy in learning-disabled people with difficult-to-control epilepsy. Seizure freedom is a realistic goal in this population.

RESUMO -Estudamos a resposta clínica ao topiramato (TPM) em crianças abaixo de 15 anos de idade. O tratamento foi iniciado com doses de 12,5 mg/kg/dia (1-7 mg/kg/dia), com aumentos semanais de 12,5 mg. .oram estudadadas 11 crianças, 9 do sexo feminino e 2 do masculino, de 3 a 14 anos de idade, com epilepsia parcial associada a etiologias diversas. Apenas um paciente teve dor abdominal intensa. Após o início do TPM, um paciente teve controle total das crises, 5 tiveram melhora maior que 75% na frequência delas, 1 paciente apresentou melhora em 50% e 3 não tiveram controle. Ao final, verificamos controle adequado das crises mesmo em quadros mais graves, com doses de até 100 mg/dia do TPM e em associação a carbamazepina, na maioria dos casos. PALAVRAS-CHAVE: topiramato, epilepsia parcial, infância.Topiramate: an experience in children with partial epilepsy ABSTRACT -Topiramate (TPM) is a new drug currently used in Brazil. We verified the clinical responses to TPM in children under 15 years-old. We started with 12.5 mg/day (1-7 mg/kg/day) and the dosis increased 12,5 mg each week. Eleven children were studied, 9 females and 2 males, from 3 to 14 years-old with partial epilepsy associated to different etiological factors. Only one patient had an intense abdominal pain. The patients had weekly or daily seizures and after began TPM 1 patient stayed free from seizures, 5 improved more than 75% in frequency, 1 patient improved more than 50% and 3 had no control. A good control of seizures was achieved with a low dosis of TPM as monotherapy and add-on therapy with carbamazepine even in severe cases.KEY WORDS: topiramate, partial epilepsy, childhood.

Epilepsy is a neurological disorder commonly seen in pediatric age. A large number of anti epileptic Drugs are now available for the treatment of this disease; although many of these drugs have good efficacy in the treatment of both partial and generalized epilepsies, their use can be associated with various endocrinological side effects. During treatment are commonly observed changes in serum levels of many hormones, such as insulin, luteinizing hormone, follicle stimulating hormone, corticotrophin and the main sexual hormones. Therefore, in epileptic patients treated with antiepileptic, it is common to observe a number of clinical problems like abnormalities in pubertal development, weight gain, gonad dysfunction, hypothyroidism, etc.Both seizures and drugs could compromise hormonal balance in epileptic patients, but it is not yet clear to distinguish their effects.The aim of this review is to focus on the main endocrinological abnormalities induced by antiepileptic drugs, in order to achieve a critical analysis of the recent researches on this topic and to offer practical advice for a better care of epileptic children.
