Relevance. Today, there exist a large number of methods for arthroscopic reconstruction of the anterior cruciate ligament, however, the return to the competitive level among athletes remains quite low. It is believed that the functional state depends on the restoration of the rotational and anteroposterior stability of the knee. Such data facilitate the search of techniques for additional stabilization of the knee, one of which is the reconstruction of the anterolateral ligament of the knee. The goal of the study was to assess the medium-term results of combined one-stage arthroscopic reconstruction of the anterior cruciate and anterolateral ligaments of the knee in athletes and the probability of their return to the competitive level. Material and Methods. In the period from 2014 to 2015, 50 patients underwent surgery. They were divided into 2 groups. Group 1 (main) consisted of 20 patients, including 10 professional athletes, who underwent the arthroscopic reconstruction of the anterior cruciate ligament, supplemented by reconstruction of the anterolateral ligament. Group 2 (control) included 30 patients (of which 10 were the professional athletes) who underwent the arthroscopic reconstruction only of the anterior cruciate ligament. Results. Group 1. 2 years after surgical treatment, 100% of patients were able to return to the preoperative competitive levels of activity. The average Tegner Lysholm score before the operation was 72.60±6.45 points, after the operation-97.40±1.18 points. The average value on the IKDC scale before surgery was 63.1±4.8%, after surgery-96.3±1.8%. Group 2. Of 30 patients, 2 years after surgery, 20 patients returned to the preoperative and competitive levels (66.7%). Of the professional athletes, 5 out of 10 patients (50%) returned to the competitive level, among amateur athletes-15 out of 20 patients (75%). The average Tegner Lysholm score before surgery was 69.6±3.5 points, after-92.1±3.9 points. The average value on the IKDC scale before surgery was 73.4±3.2%, after-90.3±3.7%. Conclusion. Medium-term results of the study showed that the onestage restoration of the anterior cruciate and anterolateral ligaments, compared with arthroscopic reconstruction of only the anterior cruciate ligament, increased the probability that the patients with high functional requirements and professional athletes would return to sports.

Background Rupture of the anterior cruciate ligament (ACL) is one of the most common sports injuries of the knee joint. Today, we have a large number of approaches to arthroscopic reconstruction of the anterior cruciate ligament that lead to successful outcomes and allow the patients to return to a significant level of activity post-operatively. Nevertheless, the return to competitions rate stays relatively low. The functional state is thought to be dependent on rotational and anteroposterior stability of the knee. These data encourage search for methods of additional stabilization of the knee joint, one of them being extra-articular tenodesis, or reconstruction of anterolateral ligament of the knee. The aim of the study To evaluate medium-term results of combined simultaneous arthroscopic reconstruction of anterior cruciate ligament and anterolateral ligament of the knee joint in sportsmen and to access the probability of return to competitions. Materials and methods The surgeries were performed in 2014-2015 in 50 patients who fulfilled the entry criteria: 20 patients (including 10 professional sportsmen) underwent arthroscopic ACL reconstruction together with reconstruction of anterolateral ligament-group 1 (main group), and 30 patients (including 10 professional sportsmen) underwent arthroscopic ACL reconstruction-group 2 (control group). Results Group 1: All patients of group 1 were able to return to the pre-operative sports level in two years after the surgery. The mean Tegner Lysholm score was 72.6 ± 6.45 (hereinafter, SE-standard error) before the surgery and 97.4 ± 1.18 after the surgery. The mean IKDC score was 63.1 ± 4.8% before the surgery and 96.3 ± 1.8% after the surgery. Group 2: 20 of 30 patients (66.7%) returned to the pre-operative level of activity and returned to competitions (if they were professional sportsmen) in a year after the surgery. Five of ten patients (50%) (professional sportsmen) returned to competitions. Fifteen of 20 patients (75%) (amateur sportsmen) also returned to competitions. The mean pre-operative Tegner Lysholm score was 69.6 ± 3.5, and the mean postoperative score was 92.1 ± 3.9. The mean pre-operative IKDC score was 73.4 ± 3.2%, and the mean post-operative score was 90.3 ± 3.7%. ConclusionThe results of the study show that more patients with higher functional demands and more professional sportsmen returned to sports. Despite the results of our and other foreign studies, a need remains for studies that will compare outcomes of ALL reconstruction with the same surgical technique in homogenous groups of patients.

Clinical outcomes of percutaneous reconstruction of anterolateral ligament in anterior cruciate ligament-deficient knee in skeletally mature patients

Background Rupture of the anterior cruciate ligament (ACL) is one of the most common sports injuries of the knee joint. The results of combined ACL and anterolateral ligament (ALL) reconstruction have shown a significant improvement in clinical outcome. Objectives To evaluate clinical outcomes of combined ACL and ALL reconstruction in ACL-deficient knee. Patients and methods This study included 20 patients aged from 21 to 45 years, who presented with ACL-deficient knee. Combined ACL and ALL reconstruction were performed to all cases. Patients were followed up for 24 months postoperatively. Patient assessment included preinjury, preoperative and postoperative subjective and objective International Knee Documentation Committee, Tegner activity scale, and Lysholm scores. Results Our study showed that the postoperative mean Lysholm knee score (94.20 ± 4.55) was significantly improved than preoperative (65.62 ± 3.42) (P<0.001) and there were 17 (85%) patients postoperatively with an excellent score. Also, postoperative mean Tegner activity score and International Knee Documentation Committee (8.22 ± 1.64 and 87.25 ± 8.71, respectively) were significantly improved than the preoperative (6.73 ± 1.21 and 52.71 ± 11.35, respectively) values (P=0.031 and P<0.001, respectively). The instrumented knee laxity test was performed using a KT-1000 arthrometer where the mean of postoperative translation (3.28 ± 0.74 mm) was significantly lower than the preoperative (11.15 ± 0.82 mm) (P<0.001) value. Conclusion Our technique of combined ACL and ALL reconstruction was found to be effective in improving subjective and objective outcomes. Also, no serious complications were noted with this operative procedure.
