Total or Near-Total Lower Eyelid Defect Reconstruction Using Malar Myocutaneous Bridge and Nasojugal Flaps and Septal Chondromucosal Graft

Abstract: As an addition to classical methods, the present novel single-staged surgical procedure with the malar myocutaneous bridge flap, nasojugal transposition flap, and septal chondromucosal graft combination provides anatomical, functional, and stable reconstruction for total or near-total lower eyelid defects.

“…Nasal mucosal grafts share a histological resemblance to the tarsoconjunctiva, containing a large number of goblet cells and subepithelial mucin glands [51], facilitating mucin substitution. These grafts provide firmness and a stable eyelid margin [52,53]. However, grafts can contain nasal hair causing conjunctival or corneal irritation [54].…”

“…Hard palate mucoperiosteal grafts offer structural support similar to the posterior lamella, but may cause discomfort post-transplantation [ 90 , 91 ]. Nasal mucosal grafts, histologically similar to tarsoconjunctiva, provide a stable eyelid margin but may cause irritation due to nasal hair [ 91 , 92 , 93 ]. Auricular cartilage grafts, known for their durability, may cause corneal irritation due to their texture and lack of secretory function [ 94 , 95 ].…”
