Total spondylectomy for solitary bone plasmacytoma of the lumbar spine in a young woman: a case report and review of literature

Abstract: Introduction Solitary bone plasmacytoma (SP) is a rare diagnosis for which the primary treatment is local radiotherapy. There is no established consensus suggesting a total spondylectomy in spinal SP. Materials and Methods We report the case of a 43-yearold woman with solitary plasmacytoma of the lumbar spine treated with complete vertebral resection. Radiographs, CT scan and MRI showed a single osteolytic lesion of the third lumbar vertebra. Further diagnostics following recommended algorithm for tumour scree… Show more

“…6 The median overall survival rate for solitary plasmacytoma of bone varies from 7.5 to 12 years, 7 and vertebral involvement has been reported as a poor prognostic factor compared with other sites of skeletal involvement. 8 The serum and urinalysis laboratory findings that characterize multiple myeloma are almost never present in a patient with a solitary plasmacytoma. Dissemination from a solitary plasmacytoma to multiple myeloma occurs within 5 years of initial presentation up to 80% of the time.…”

“…SBP can be diagnosed by biopsy demonstrating monoclonal plasma cell infiltration of a single lesion. The median age at diagnosis of SBP is 10 years younger than that of MM[ 3 ]. To date, the treatment choices of tumors in the upper cervical spine are controversial, especially in terms of the surgical approaches, extent and reconstruction.…”

“…SBP is a rare diagnosis for which the primary treatment is local radiotherapy. The exact role of surgery for SBP remains unclear[ 3 - 5 ]. Although MM is a common disease when compared to other plasma cell neoplasms, SBP in the vertebral body is a rare condition with little published literature[ 3 ].…”

“…Metastatic disease of the cervical spine is associated with a poorer prognosis than the thoracic or lumbar spine, in addition to surgery in this region having a higher morbidity . The exact role of surgery for spinal MM remains unclear . Age and subtype affect survivorship and skeletal‐related events respectively .…”
